芬太尼-安定与吗啡-安定复合麻醉对心血管功能的影响

摘　　要：本文报道60例心脏直视手术患者,随机以芬太尼-安定和吗啡-安定复合麻醉分为两组,芬太尼及安定剂量分别为25.6±3.6μg/kg及0.61±0.17mg/kg。吗啡及安定剂量分别为0.8±0.12mg/kg及0.72±0.08mg/kg。应用肺漂浮导管及热稀释心排血量计测量麻醉前、后的血流动力学改变,比较两种麻醉对心血管功能的影响。结果:两组麻醉后MAP、HR、SVR、MPAP有不同程度下低,这对减轻心脏负荷及减少心肌耗氧有利。然而CO、CI亦降低。芬太尼组的降低轻微,无临床意义,吗啡组的降低较显著。提示芬太尼-安定优于吗啡-安定麻醉。Sixty patients undergoing open-heart surgery were divided into two groups. Fentanyl group,30 cases, received fentantl(25.6±3.6ug/kg) and diazepam(0.61 ±0.17mg/kg).Morphine groups, 30 cases, received morphine (0.8 ±0.12mg/kg) and diazepam (0.72±0.80mg/kg). Hemoaynamie changes were measured using flow directed pulmonary artery catheter before and during anesthesia, The hemoaynamic changes in two groups were compared. The results showed that there were the decreases in MAP, HR, SVR, MPAP in both of two groups after anesthesia, it may be beneficial to cardiac protection. However, there were also decreases in cardiac output and caraiac index after anesthesia in the two groups but it was remarkable in morphine group (P<0.01) and less significmt in fentanyl group (P>0.05), indicating that fentanyl or morphine combined with diazepam cam produce cardiovascular depression but it was more serious in morphine than in fentanyl, It is concluded that fentanyl anesthesia combined with diazepam is better than morphine combined with diazepam.
