动脉栓塞法治疗甲状腺机能亢进症的副作用及并发症探讨

摘　　要：目的探讨甲亢介入治疗的副作用与并发症及防治。方法对61例甲亢病人的异常甲状腺上、下动脉进行超选择性插管,并用聚乙烯醇微粒及明胶海绵颗粒进行栓塞。结果所有病例均出现颈部不适,53例(86.9%)出现颈部疼痛及不同程度吞咽困难,11例(18.0%)出现发音困难,声音嘶哑,17例(27.8%)出现牙疼,18例(29.5%)出现皮肤瘀斑,1例(1.6%)患者出现脑梗塞,1例(1.6%)患者出现甲状腺危象,无1例病人出现甲状腺机能减退或甲状旁腺机能减退。结论动脉栓塞法治疗甲亢有一定的副作用及并发症,严格操作规程,术后严密观察病人反应,可防止及控制一些严重并发症的发生。Objective To study the side -effect and complic ations of interventional therapy of hyperthy-roidism and the prevention and treatment.Methods Super selective catheterization fo llowed by embolization with PVA microspheres and Gelfoam partic les were performed within the abnorm al thyroid superior and inferior arteries of in patients with hyperthyroidism.Results All the patients felt discomfort in t heir neck,53patients felt painful in their neck and had difficulty in sw allowing.Other 11patients had diff iculty in pronunciation or hoarse pr o-nunciation,17patients had toothac he,18patients have purpuric skin ra sh,one patient had brain infarct,an d one patient has life-threatening thyro toxicosis.No hypothyroidism and hypoparathyroidism are observed.Conclusion There are some side -effect and compl ications in the thyroid artery embol ization for hyperthyroidism.Careful ma-nipulation during the procedure and close post-intervention care can pr event and manage some serious complications.[
