尖锐湿疣治愈后皮肤及毛囊人乳头瘤病毒检测

摘　　要：目的:研究尖锐湿疣(CA)患者经治疗后原患处外观正常的皮肤和周围的毛发毛囊是否还携带人乳头瘤病毒(HPV)及其与复发的关系。方法:CA患者常规微波治疗后2～3周,取治疗后外观正常的原患处皮肤黏膜和周围的毛发毛囊,进行HPV-DNA-PCR检测。随访3个月,观察复发情况。结果:治疗后皮肤黏膜HPV检测阳性者中,HPV6/11型为15/71例(21.13%),HPV16/18型为5/71例(7.04%)。总复发率:阳性患者为65.00%,阴性患者为15.69%,P<0.005,二者间差异有显著性。患处周围的毛发毛囊处HPV检测阳性者中,HPV6/11型为8/39例(20.51%),HPV16/18型为2/39例(5.13%)。总复发率:阳性患者为60.00%,阴性患者为20.68%,差异亦有显著性,P<0.01。结论:HPV可潜伏于治疗后外观正常的皮肤黏膜和周围毛发毛囊处,潜伏的HPV作为病毒储存库与CA的复发相关。提示我们在临床中对有HPV潜伏的患者要进行随访观察和亚型检测分析,尤其是有癌变可能的高危型携带者。Objective: To investigate of DNAs of HPV presenting in the skin and mucous membrans with normal appearance and in the ambient hair follicles after treatment of condyloma acuminatum (CA) and discuss the relation with the CA recurrence. Methods: HPV DNA was detected in skin and mucous membrane samples 2 to 3 weeks after treatment to CA by using polymerase chain reaction. Patients were followed-up to observe the recurrence for 3 months. Results: Type 6/11 of HPV was detected in 15 out of 71 (21.13%) and type 16/18 of HPV was detected in 5 out of 71 (7.04%) in skin and mucous membrane samples. The relapse rate was 65% in the positive detection group of HPV DNA and 15.69% in the negative group. There was a significant statistical difference (P < 0.005) of the relapse rate between the positive and negative groups of HPV DNA detection. Type 6/11 HPV was detected in 8 of the 39 (20.51%) and type 16/18 HPV was detected in 2 of the 39 (5.13%) in hair samples. The relapse rate was 60% in the positive group of HPV DNA detection and 20.68% in the negative detection group (P < 0.01). Conclusion: HPV still remained in the skin and mucous membrane with normal appearance after treatment to condyloma acuminata. HPV is in the endogenous reservoir related to the recurrences of CA. It is very important to follow-up and monitor HPV subtypes. The positive detection of high risk HPV-DNA suggests. Some risk for developing carcinoma.[
