云克(^(99)Tc-MDP)联合慢作用药物(DMARDs)治疗类风湿关节炎的临床研究(附56例分析)

摘　　要：目的　观察云克 (99锝 -亚甲基二膦酸盐 ,99Tc- MDP)、甲氨蝶呤、柳氮磺吡啶联合治疗类风湿关节炎(RA)的临床近期疗效及其安全性。方法　 5 6例类风湿关节炎活动期患者随机单盲分为两组 ,其中云克治疗组 (治疗组 ) 2 8例 ,用云克、甲氨蝶呤、柳氮磺吡啶治疗 ;非云克治疗组 (对照组 ) 2 8例 ,用万络、甲氨蝶呤、柳氮磺吡啶。疗程 4周 ,分别观察治疗前后临床指标及炎性实验室指标的变化 ,并记录服药后不良反应。结果　治疗组的总体有效率为96 .4 % ,对照组的总体有效率 82 .1%。经 Ridit分析 ,两组间有显著性差异 (P<0 .0 5 )。两组治疗前后血清肿瘤坏死因子 - α(TNF- α)水平的降低水平比较有显著性差异 (P<0 .0 5 )。不良反应方面 ,治疗组和对照组间的副作用相似 ,表现为头痛、眩晕、恶心 ,两组副作用的发生率比较差异无显著意义。结论　联合云克治疗类风湿关节炎近期疗效满意、起作用早、副作用少 ,远期疗效有待进一步观察。Objective ]To observe the clinical effect and safety of ^(99)Tc-MDP combined with DMARDs in the treatment of rheumatoid arthritis.To observe the clinical effect and safety of ^(99)Tc-MDP combined with DMARDs in the treatment of rheumatoid arthritis. 56 patients with rheumatoid arthritis were randomly divided into two groups^(99)Tc-MDP group and non-^(99)Tc-MDP group).With the addition of (99)Tc-MDP in ^(99)Tc-MDP group and Rofecoxib in non-^(99)Tc-MDP group,respectively,all patients in study were treated with MTX and SASP at the same time.Data about their clinical effect and laboratory results were noted. The effectual rate of(99)Tc-MDP group was 96.4%,while that of the non-^(99)Tc-MDP group was 82.1%.Ridit analysis showed that there is significant difference between two groups(P<0.05).The comparison of decrease of TNF-α level before and after the treatment showed significant difference between the two groups(P<0.05).In both groups,side effects were recorded in 7.14%(2/28)of the patients without significant difference. Conclusion Combination of ^(99)Tc-MDP and DMARDs has turned out to be an effective and reliable method in treatment of rheumatoid arthritis with few side effects.However,it needs more time to observe the long-term clinic effect of this method.
