蛇床子素在急性肾损伤兔体内的药动学研究

摘　　要：目的 :对 18例肾功能正常与受损兔iv蛇床子素 (osthole ,Ost)的药动学进行研究。方法 :按生化指标BUN值和Cre值作自身对照并进行肾病理切片 ,分成 3组 (每组n =6 ) :第 1组 (肾功能正常组 ) ,第 2组 (肾功能轻度受损Ⅰ组 ) ,第 3组 (肾功能严重受损Ⅱ组 )。第 1,2组ivOst溶液 10mg/kg ,第 3组为 7 5mg/kg。HPLC法测定血浆中Ost的浓度。结果 :与正常组相比 ,肾受损Ⅰ、Ⅱ组的表观分布容积 (Vc)均无显著性改变 ,肾轻度受损I组的各药动学参数亦无显著改变 ;但肾受损II组的AUC、t1/ 2 β及CLs较正常组显著改变 ,其中AUC升高到正常组的 1 7倍 ,CLs比正常组降低了 4 2 % ,而t1/ 2 β较正常组延长了 1 5倍。结论 :当肾衰较重 (Cre升高 4 2倍时 )应调整给药方案。AIM:The pharmacokinetics of osthole was studied in 18 rabbits with normal and impaired renal function after intravenous administration. METHOD:Eighteen New Zealand rabbits were randomly divided into three groups(for each group,n=6),based on their BUN,and Cre value,and their kidneys were taken out to make microscopic sections by HE colouring to confirm the different stages of renal failure:group 1 (normal renal function);group 2 (mild renal impairment);group 3 (severe renal impairment). All rabbits in group 1,2 received dose of 10 mg/kg and 3 received dose of 7.5 mg/kg,respectively. The osthole concentration of plasma was determined by HPLC. RESULT: The apparent volumes of distribution were not significantly different from each other in the three groups. AUC was 1.7 fold,t_ 1/2β was 1.5 fold higher and CLs was 42% lower in group 3 than those in group 1. CONCLUSION:The results suggests that the therapentical protocol of osthole for the patients with severe renal failure in clinic should be adjusted.
