短期胰岛素强化治疗改善初发2型糖尿病胰岛功能疗效观察

摘　　要：目的探讨短期胰岛素强化治疗,对改善伴明显高血糖的初诊2型糖尿病患者的胰岛β细胞功能和血糖控制的影响。方法采用自身前后对照,对35例空腹血糖(FPG)>11.1mmol/L的初诊2型糖尿病患者进行3周胰岛素强化治疗,观察其治疗前后FPG、餐后2小时血糖(2hPG)、Homa胰岛素分泌指数(Homa-β)、Homa胰岛素抵抗指数(Homa-IR)的变化。结果治疗3周后,FPG、2hPG明显下降,Homa-β明显上升,Homa-IR明显下降(P<0.05)。结论对伴有明显高血糖的初诊2型糖尿病患者,短期胰岛素强化治疗有快速稳定血糖和显著改善胰岛β细胞分泌功能的作用,同时对胰岛素抵抗也有一定的改善。Objective To study the effect of short-term intensive insulin treatment on β-cell function and glycemic control in newly diagnosed type 2 diabetic patients with severe hyperglycemia. Methods 35 newly diagnosed type 2 diabetic patients with FPG>11.1mmol/L were treated by 3 weeks intensive insulin treatment. The results of FPG, 2hPG, Homa-β, Homa-IR were compared before and after the intensive insulin treatment. Results After 3 weeks treament, FPG, 2hPG, Homa-IR, were significantly decreased, and Homa-βsignificantly was increased (P<0.05). Conclusion The excellent glycemic control and improvement of β-cell function can be induced by short-term intensive insulin treatment in newly diagnosed type 2 diabetic patients with severe hyperglycemia.
