改良Pemberton髋臼成形术治疗发育性髋脱位

摘　　要：目的 :总结和探讨采用改良Pemberton髋臼成形术治疗发育性髋关节脱位 (DDH)的经验。方法 :1992年至 2 0 0 1年 ,4 2例 5 3髋手术中包括①术前牵引 ,内收肌、髂腰肌切断 ;②术中股骨粗隆下短缩旋转截骨 ;③髋关节切开中心性复位 ;④改良Pemberton髋臼成形术 ,恢复头臼匹配关系 ;⑤复位后位置的维持 ,一单髋人字石膏外固定 3～ 6周。结果 :术后随访 1～ 9年平均 3年 ,优良率 90 .6 %。结论 :Pemberton髋臼成形术是治疗发育性髋脱位的一种有效方法。Objective:To conclude and explore the experiences of treating dysplasia dislocation of hip (DDH).Method:from 1992 to 2001,42 patients(53hip) were operated by this way.Including:①preoperative traction and adductomy and iliopsoasoctomy.②correcting femoral neck anteversion angle by subtrochanteric shortening and derotational osteotomy.③central reduction of the hip joint by incising hip joint capsule.④restoring the matchrelationship of femoral head and acetabular by improved Pemberton's osteotomy.⑤maitaining the unnormal hip joint at “人”—shaped plaster fixation after operation.Results:All patients were folllwed up 1 to 9 years after operation ,averaging 3 years.excellent and good percentage is 90.6%.Conclusions: Improved pemberton acetabuloplasty is a very effective treatment of dysplasia dislocation of the hip (DDH).
