摘 要:目的测算贵州省2001年不同经济水平的省地县乡村五级计划免疫服务的实际成本 ,了解计划免疫 (计免 )成本状况及构成,探讨成本构成及各级政府财政补助水平对计免服务的影响。方法用分层随机抽样方法选点 ,在全省省地县乡村抽取的17个调查点进行现场调查并填写调查表 ,通过数据统计测算计免成本、政府补助及构成状况。结果省地县乡村各级计免服务总成本分别为1147000元、205000元、79000元、29500元、688元 ;各级直接成本占总成本的构成比为64.69%、73.33%、75.00 %、88.12 %、100 % ;直接成本中各级人员费用所占比例为47.84 %、70.86 %、63.33%、76.68%、100%。而业务费用比例为35.44%、13.91 %、18.34%、6.39 %和0 ;政府财政投入主要是人员费用 ,各级业务费用缺口比例分别为省级4.97 %、地级68.96 %、县级11.68% ,乡级除一个调查乡外 ,其余调查乡无业务开展费用 ;所有调查村级无业务开展费用。结论贵州省计免成本测算中 ,直接成本中的人员费用除省级较合理外 ,县乡村的人员费用较高 ;直接成本中的业务费用所占比例较低 ,乡村级业务费用很少或无业务费用 ;提示政府财政对计免服务投入不足 ,尤其是乡村级财政投入很少或无投入。财政投入的严重匮乏 ,影响乡村级人员素质的提高和基层卫生人员队伍的稳定 。Objective To account the actual cost of Expand Programme on Immunization(EPI)in5health wet grades that are province、prefecture、county、township and village in the light of different economical developing levels in Guizhou Province,understand the cost and structure of EPI,study cost structure and government financing investment affected to EPI services.Methods17research places were selected by random sampling and investigated through the field survey and filling out the questionnaires.Cost of EPI and government financing compensation and structures were acˉcounted according to the data analysis.Results Whole cost in5grades of health wet for the province and prefecture and county and township and village was1,147,000,205000,79000,29500and688RMB,respectively.The proportion of the for direct cost account whole cost was64.69%,73.33%,75.00%,88.12%and100%,respectively;and proporˉtion of EPI staff fare for direct cost account was47.84%,70.86%,63.33%,76.68%and100%,respectively.Proportion of working fare for Direct Cost account was35.44%,13.91%,18.34%,6.39%and there was not working fare in vilˉlage level.The government financing investment mainly defrayed the staff fee of EPI and each health wet grade was short of the surveillance and working fare.The proportion of shortage was4.97%in province and68.96%in prefecture and11.68%in county,and except one survey town there were no working fare in township level.There were no working fares in all investigation villages.Conclusion It is shown that staff fare proportions of direct cost of EPI of in county and township and village levels are higher except for the province levels is in reason,but the absolute value is quite low.The proportion of working fare for direct cost in different health wet grades is lower and there is no working fare or very fewin township and village levels.It means that the government financing investment can not satisfy the needs of EPI services because of the economic backwardness in Guizhou Province.It is suggested that financing investment of go
