2001年广西伤寒监测结果分析

摘　　要：目的及时全面了解、掌握广西伤寒主要流行菌型变化动态和传染源。方法在广西桂林市及其辖区7个县开展伤寒监测 ,对发热疑似病人进行病原学、血清学检测 ;并对密切接触者、病人病后带菌调查 ;水体及疫点环境调查。选取桂林市10家医院作为哨点医院。结果2001年8个监测点发病数为2889例 ,占广西病例总数的52.28% ,发病率为80.55/10万。哨点医院病原学监测 ,阳性率22.28%,甲型副伤寒沙门氏菌占95.34 % ,伤寒沙门氏菌仅占4.66 %。监测点血培养阳性率15.64% ,甲型副伤寒沙门氏菌占92.69% ,伤寒沙门氏菌占7.31%。结论我区目前流行的主要菌型以甲型副伤寒沙门氏菌为主 ,传染源仍广泛存在 ,防治形势不容乐观。加强监测和有效降低医疗费用 ,是管理、控制传染源的重要措施。Objectives To determine the infection sources of typhoid and paratyphoid,as well as the shift of predominant causative agents that cause epidemics in Guangxi.Methods A surveillance system of typhoid and paratyphoid fever has been established in Guilin and its surrounding areas.Under this system,cultures and serology tests were done in suspected patients with fever.Investigations of close contacts,carriers,water and enˉvironment were conducted at the epidemic sites.10medical facilities were entitled sentry hospitals in Guilin.Results2889cases of typhoid and paratyphoid fever were captured at8surveillance sites in2001,accounting for52.28%of the total number of cases across Guangxi.The incidence rate was80.55per100000populations.The positive rate of culture was22.28%at sentry hospitals.92.58%of the isolates were S.pratyphi A and4.66%were S.tyhpi only.The positive rate of blood culture at the surveillance sites was15.64%,with S.paratyphi A accounting for92.69%and S.thphi for7.31%.Conclusion The situation of prevention is still austere because S.pratyphi has become the predominant causative agent that causes epidemics presently,and the infection sources have been found widely.Further surveillance and decrease of medical charge are considered as important measures of management of patients and carriers.
