摘 要:目的 探讨腹膜透析 (PD)充分性对营养状态的影响。方法 选择透析不充分的 1 9例持续不卧床腹膜透析患者 ,采用腹膜平衡试验 (PET) ,并根据PET结果对患者原有的PD方案进行适当的调整 ,分别观察调整前及调整后 3个月的透析充分性指标 :即临床指标、尿素清除率 (KT/V)和肌酐清除率 (Ccr)。同时 ,评估营养指标 :血清白蛋白 (Alb)及血红蛋白 (Hb)。结果 调整透析方案前 ,在透析不充分的情况下 ,患者的营养指标低下 ,根据PET结果对原有的PD方案进行适当的调整后 ,1 9例患者中有 1 8例透析达到充分指标 ,Alb与Hb值与调整前比较均有显著性提高 (P <0 0 1 ) ,余下的 1例患者透析仍不充分 ,故改为血液透析治疗。结论 充分透析可改善PD患者的营养状态。PET可指导临床制定合理的PD方案 ,从而使透析达到充分。Objective To investigate the effect of peritoneal dialysis (PD) adequacy on nutritional status. Methods 19 cases of continuous ambulatory peritoneal dialysis patients who couldn't get adequate dialysis were selected. Their PD schemes were adjusted properly according to their results of peritoneal equilibration test (PET). Adequacy parameters including clinical parameters, KT/V and Ccr were observed at 0 and 3 months. the serum albumin and hemoglobin (Hb) were also measured. Results BeforePD schemes were adjusted the patients had low albumin and Hb levels because the couldn't get adequate dialysis. 18/19 patients got adequate dialysis after adjusting their PD schemes according to their results of PET, and the levels of albumin and Hb were significantly increased (P<0.01). The case who couldn't get adequate dialysis changed to hemodialysis. Conclusion Adequate dialysis could improve the nutritional status of PD patients. PET might be helpful to make a reasonable PD scheme and adequate dialysis.
