摘 要:目的 研究谷氨酰胺对内皮细胞功能的影响,探讨其在呼吸衰竭合并营养不良患者治疗中的作用。方法 将43例慢性阻塞性肺病(COPD)患者分为谷氨酰胺组(Glu)和非谷氨酰胺组(NGlu),按1.5 REE由营养师指导饮食10 d,Glu组将蛋白质减少30 g,以L-谷氨酰胺30 g取代,10 gtid。前后均采血检测NO、NOS、vWF及ET-1。结果 Glu组NO、ET-1与NGlu组有显著差异(P<0.01),两组vWF也有明显差异(P<0.05),NOS无显著差异。结论 谷氨酰胺对cOPD呼吸衰竭合并营养不良患者内皮细胞功能有显著改善。Objective To study the effect of glutamine on the endothelial function and discuss it's efficacy in the treatment of respiratory failure complicated with malnutrition. Methods Forty-three COPD patients were divided into two groups: glutamine (Glu) group and no glutamine ( NGlu) group. All patients abided by 1. 5 REE 10 d under the guidence of the nutritionists. The Glu group replaced 30 g protein with 30 g L- glutamine, 10 g tid. NO,NOS, vWF and ET-1 were determined before and after the experiment. Results There was significant difference in NO,ET-1 and vWF,but no obvious difference in NOS between the Glu group and NGlu group. Conclusion L- glutamine can obviously change the endothelial function of COPD respiratory failure patients complicated with malnutrition.
