腹主动脉骑跨栓塞治疗体会

摘　　要：目的　总结和分析腹主动脉骑跨栓塞的治疗经验和教训。方法　 1990～ 2 0 0 3年 ,经双侧股动脉逆行导管取栓 15例 ,腋动脉双侧股动脉转流术 1例 ,主动脉双侧股动脉叉状人造血管旁路移植 2例 ,未手术 1例。结果　死亡 4例 (含未手术 1例 )。截肢 3例 4肢 ,截趾 2例 5趾。 1例足下垂。其他病例治疗成功。结论　提高对腹主动脉骑跨栓塞的认识 ,早期诊断、早期手术 ,尽早恢复肢体远端血流通畅 ,同时注意原发病和并发症的治疗 。Objective To summarize and analyze the treatment experience of aortic saddle embolism. Methods Nineteen patients were included,including 15 patients operated via both femoral; 1 patient operated via armpit and both femoral, 2 patients operated by artificial vascular transplant,and 1 patient was not operated. Results Four patients died(including 1 not operated); 3 patients were amputated 4 limbs, 2 patients cut 5 toe of 2 feet; and 1 patient got foot sag. Others were successfully cured. Conclusion Uderstanding the disease, early diagnosis, early operation and early restoring of blood flow in extremities and management of the primary affection and complication are important to decrease the mortality and the disablity in the patients.
