天津海河医院SARS患者的临床检验结果分析

摘　　要：目的 :分析严重急性呼吸综合征患者血液学指标的改变特点,为临床诊治提供实验依据。方法 :对2003年5～8月35名SARS患者的外周血血细胞,生化等指标进行检测,同时对患者的各项指标进行发病早期,中期和恢复期的动态观察;对16例疑似患者外周血血细胞,生化等指标进行检测与SARS患者发病早期的化验结果进行比较。结果 :发病早期,35例SARS患者白细胞在正常范围内的为54.3%,低于4.0×109/L为40 %;淋巴细胞绝对值低于1.0×109/L为78.0%,正常范围的为22.8 %;RBCHGBPLAT均正常。在发病早期心肌酶指标 ,谷草转氨酶(AST)乳酸脱氢酶(LDH)浕 -羟丁酸脱氢酶(HBDH)肌酸激酶(CK)显著增高,与疑似患者相比,差别有统计学意义。结论 :SARS患者的发病早期LYM,白细胞的显著降低;发病早期的ASTCK、LDH和HBDH显著增高,对SARS患者和疑似患者的确诊,有重要的临床意义。Objective: To analyze the changes of some hematologic features in patients with severe acute respiratory syndrome (SARS).Methods:35 cases blood cells and biochemical index were determined, and the levels were compared. The changes of these features in different phases of SARS also were investigated.Results: Among the 35 SARS cases, the initial blood counts showed leukopenia(total white-cell count <4.0×109/L) in 40percent of patients. 78% of the patients had moderate lymphopenia(absolute lymphocyte count<1.0×109/L).There were several abnormalities in SARS patients. Serum levels of aspartate aminotransferase(AST), Creatine kinase(CK), lactate dehydrogenase(LDH), and HBDH were significantly higher than those of the suspected cases.Conclusion: Leukopenia, moderate lymphopenia, elevated levels of AST, CK, LDH and HBDH are prevalent in the early phase of the illness. These laboratory findings can alert medical practitioner to define the possibility of SARS.
