哮喘儿童焦虑情绪与唾液SIgA测定

摘　　要：【目的】　通过评定哮喘儿童焦虑情绪、观察焦虑与唾液SIgA的关系 ,探讨改善哮喘儿童免疫、减少哮喘发作的新方法。　【方法】　采用汉密顿焦虑量表 (HAMA)对 40例哮喘患儿进行评定、测量皮肤电阻和唾液SIgA ,并与 2 6例健康儿童对比。　【结果】　哮喘组HAMA分值明显高于对照组 (分别为 12 .0 3± 6.0 1,4.96± 3 .89,P<0 .0 1) ,SIgA明显低于对照组 [分别为 ( 169.14± 13 6.19)mg/L ,( 3 41.2 3± 182 .89)mg/L ,P <0 .0 1] ,皮肤电阻值明显高于对照组 (P <0 .0 1)。与哮喘无焦虑组比较 ,哮喘焦虑组SIgA明显降低而皮肤电阻明显增高 (P <0 .0 5 )。哮喘发作组HAMA分值明显高于哮喘缓解组 [分别为 ( 10 .71± 4.65 ) μΩ ,( 5 .5 3± 2 .2 4) μΩ ,P <0 .0 1]。 　【结论】　哮喘儿童存在较明显的焦虑情绪障碍 ,其体液免疫功能减低 ,副交感神经功能亢进。与无焦虑的哮喘儿童比较 ,有焦虑的哮喘儿童体液免疫功能较低 ,副交感神经兴奋性较高。哮喘儿童发作期焦虑情绪多于缓解期。By assessment of anxiety in asthmatic children and observing the relationship between anxiety and SIgA content, to research new methods of improving asthmatic children’s immune condition and relieving asthmatic attacks. Hamilton anxiety scale (HAMA), skin resistance, salivary SIgA immuno radiometry assay were measured in 40 asthmatic children, comparing with 26 healthy children. HAMA score in asthma group was higher than in control group ( P <0.01). The SIgA level and skin resistance were lower and higher in asthma group than control group( P <0.01),the same in asthma group with anxiety and asthma group without anxiety ( P <0.05). HAMA score in attack group was much higher than in convalescent group ( P <0.01). [Conclusions] Asthmatic children had more obvious anxiety disorder. Humoral immune function decreased and the function of parasympathetic nervous system increased in asthmatic children. Humoral immune function was lower in anxious asthmatic children. Parasympathetic nervous excitement in asthmatic children with anxiety was higher than in asthmatic children without anxiety. Asthmatic children in attack had more anxiety than asthmatic children in convalescence.
