摘  要:目的 探讨极低出生体重儿脑室内出血 (IVH)的临床特点和危险因素。方法 对新生儿重症监护病房 48例并发IVH的极低出生体重儿与同期住院治疗、曾经头颅B超检查的 77例极低出生体重儿进行回顾性对照分析。比较两组患者的临床特点 ,采用非条件Logistic回归进行多因素分析并计算OR值及OR的95 %可信限 ( 95 %CI)。结果 极低出生体重儿IVH的发病率为 3 8 4%。多因素非条件Logistic回归分析显示 :围产期窒息 (OR =6 3 79,95 %CI=1 192~ 3 4 14 4)、机械通气时间 (OR =8 3 74,95 %CI=2 5 61~ 2 7 3 88)和苯巴比妥治疗 (OR =0 0 81,95 %CI=0 0 2 3~ 0 2 68)为极低出生体重IVH的独立相关因素。结论 IVH在早产儿中发生率高 ,围产期窒息、机械通气时间和苯巴比妥治疗与极低出生体重儿IVH的发生有密切关系。Objective To identify the clinical characteristics and risk factors of intraventricular hemorrhage (IVH) in the very low birth weight infants. Methods A case-control study was conducted in 125 very low birth weight (VLBW) infants. The case group was those neonates with IVH, and the control group was those without IVH. Non-conditional logistic regression analysis was performed to determine the risk factors for IVH. The odds ratios (OR) and their 95% confidence intervals (95%CI) were estimated. Results The incidence rate of intraventricular hemorrhage was 38.4% in VLBW infants. By non-conditional logistic regression analysis, perinatal asphyxia (OR=6.379, 95% CI=1.192~34.144), duration of mechanical ventilation (OR=8.374, 95% CI=2.561~27.388), treatment with phenobarbital (OR=0.081; 95% CI=0.023~0.268) were significantly associated with IVH independently. Conclusions Perinatal asphyxia, duration of mechanical ventilation and treatment with phenobarbital are significantly correlated with the occurrence of IVH in VLBW infants.
