冠心病患者开胸非心脏手术157例临床分析

摘　　要：目的 :探讨冠心病患者开胸非心脏手术心血管并发症和病死率的关系。方法 :对术前临床诊断有冠心病的择期开胸非心脏手术 15 7例进行临床分析。结果 :15 7例患者术后心律失常发生率 86.62 % ,急性心肌梗死发生率 2 .5 5 % ,栓塞发生率 3 .82 % ,总病死率 5 .10 %。结论 :冠心病患者开胸非心脏手术风险相对较大 ,但加强围手术期监护 ,缩小手术范围和应用支持、治疗药物 ,可减少术后心血管并发症和病死率。Objective To discuss cardiovascular complications and motality rate of patients undergoing thoracotomy for coronary heart disease.Methods One hundred fifty-seven cases of clinically diagnosed coronary heart disease receiving thoracotomy were analysed.Results The incidence of cardiac arrythmia was 86.62%,acute myocardial infarction was 2.55% and embolism was 3.82%.Total motality rate was 5.10%.Conclusion The risk is relatively higher in patients undergoing thoracotomy for coronary heart disease.The cardiovascular complications and motality rate can be reduced by enhancing perioperative mornitoring,reducing operative range and using supportive and curative drugs.
