术前动脉插管化疗对膀胱移行细胞癌c-myc蛋白表达及预后的影响

摘　　要：目的 :探讨术前腹壁下动脉插管化疗对膀胱移行细胞癌c myc蛋白表达及预后的影响。 方法 :对化疗组膀胱癌患者 4 4例先行盆腔区域化疗 ,再行开放手术 ;对照组 6 8例行单纯开放手术。采集肿瘤标本 ,行c myc蛋白免疫组化染色 ,检测癌基因蛋白的表达 ,同时比较随访资料。结果 :化疗组c myc蛋白表达阳性率为4 5 .5 %,对照组为 6 9.1% ,两组比较P <0 .0 5 ,术前动脉插管化疗降低T2 ～T4膀胱移行细胞癌的c myc蛋白表达阳性率。化疗组 3年复发率为 2 0 .5 % ,5年生存率为 79.5 % ,10年生存率为 5 2 .3% ;对照组 3年复发率为 5 8.8% ,5年生存率为 4 7.1% ,10年生存率为 2 5 .0 %。结论 :术前动脉化疗降低膀胱移行细胞癌c myc蛋白表达 ,提高膀胱移行细胞癌患者的生存率 ,降低复发率。Objective:To observe the influence of preoperative arterial chemotherapy on c-myc oncoprotein expression in transitional cell carcinoma of bladder and the prognosis of bladder cancer patients.Methods:112 patients with an initial diagnosis of bladder cancer were investigated. 44 surgical specimens comes from the patients who underwent preoperative arterial chemotherapy ;68 specimens came from mere operation patients; 8 specimens from human normal bladder tissue. all specimens were stained by SP immunohistochemical method to detect the expression of c-myc oncoprotein.Results:The expression of c-myc oncoprotein was closely associated with the clinical stages(P< 0.05 ); the c-myc oncoprotein positive rate was 45.5 % in the group of preoperative arterial chemotherapy; and it was 69.1 % in contrast group; c-myc oncoprotein of the normal bladder tissue was not expressed. The recurrence rate of three years, survival rate of five years and ten years were 20.5 %, 79.5 % and 52.3 % respectively in preoperative arterial chemotherapy group; and those were 58.8 %, 47.1 % and 25.0 % respectively in constrast group.Conclusions:Preoperative arterial chemotherapy can decrease the c-myc oncoprotein positive rate in transitional cell carcinoma of bladder , increase the survival rate and decrease the recurrence rate of bladder cancer patients.
