同种异体黑素细胞移植治疗白癜风的进一步探索

摘　　要：目的:通过临床试验深入探索同种异体黑素细胞移植治疗白癜风的疗效和主要影响因素。方法:在没有任何配型、没有使用免疫抑制剂情况下,采取黑素细胞悬液疱内注射和黑素细胞膜片移植两种方法,对12例稳定期白癜风患者移植同种异体黑素细胞。结果:12例患者接受移植后没有特殊不适。移植后异体细胞移植部位明显发红,90d左右逐渐消退,有的为轻微黄褐色;至180d时87.5%(7/8)的患者色素未恢复,12.5%(1/8)的患者有轻微色素恢复。自体疱皮移植部位在移植后10～13d开始有色素恢复,至180d时色素恢复明显。取移植后发红的移植区皮肤行组织病理检查显示有炎性细胞浸润。结论:异体黑素细胞移植后存在免疫排斥,因此移植效果较差。Objective:To explore the therapeutic effect of transplantation of pure cultured allogeneic melanocytes to treat vitiligo in clinical trial.Methods:Twelve vitiligo volunteers enrolled allogeneic melanocyte transplantation trial,while the donors and the recipients had no genetic connections,their major histocompatibility complex(MHC)was not matched,and no immunosuppressive agents were administered before transplantations.The purified allogeneic cultured melanocytes were transplanted to the depigmented areas by injection of melanocyte suspension or melanocyte membrane graft.The autologous blister skin transplantation was carried out at the same time as a control.The results were followed up for180days.Results:The autologous transplantated areas began to be repigmented after10～13days and were markedly repigmented on day180.The allogeneic melanocyte transplanted areas showed slightly swollen and markedly erythematous.The inflammation in8patients faded gradually after90days,and only one of them showed poorl repigmentation on day180.There were inflammatory reactions in the erythematous areas shown by histopathologic examination.Conclusions:Allogeneic melanocyte transplantation results in poor repimentation under the above conditions.The clinical and histopathologic results suggest immunological rejection be existed.
