摘  要:目的 观察cGMP在热负荷慢性心衰对血流动力学中的影响 ;方法  2 5只新西兰兔作冠状动脉结扎 (CAL) ,另外 5只仅作开胸 ,不作结扎。冠脉结扎 8周后 ,2 5只兔随机分为对照组 (n =5 )热负荷组 (n =5 ) ,热负荷 +左旋硝基精氨酸甲酯 (L -NAME)组 (n =5 ) ,常温 +L -NAME组 (n =5 ) ,常温 +左旋精氨酸 (L -Arg)组 (n =5 ) ,并测定血流动力学、血浆NOS、cGMP。将心衰动物放置在环境温度 42℃ ,待肛温达到 42℃ ,维持 15~ 2 0min ,热负荷结束后 ,将动物移置到常温环境下 ,分别测定热负荷终止后的 2、4、8、2 4h的血流动力学及血浆NOS、cGMP。结果 ⑴所有手术组动物血流动力学与假手术组血流动力学相比较差异具有非常显著性 ( P <0 0 1)。⑵热负荷组 2 4h后血流动力学与对照组比较有改善 ;热负荷 +L -NAME组血流动力学与对照组比较无改善 ;常温 +L -Arg组血流动力学与对照组比较有改善。⑶手术组心衰血浆NOS、cGMP比假手术组高 ,P <0 0 1。热负荷慢性心衰2 4h后血浆NOScGMP比对照组、热负荷前非常显著升高 ,P <0 0 1。常温组L -Arg血浆NOS、CGMP高于对照组。结论 热负荷作用于慢性心衰后有保护心肌作用 ,这种保护作用与改善血流动力学 ,主要与热负荷激活NOSObjective To observe the effect of heat stress on plasma cGMP and hemodynamics in animals with chronic heart failure. Methods Coronary arteries of twenty-five rabbits were ligated to set up the animal model of heart failure and other five rabbits only received thoractomy as sham group. Eight weeks after coronary artery ligation, they are randomly divided into heat stress (HS) group, control group, HS+L-NAME group, and non-HS+L-NAME group and non-HS+L-Arg group, each group containing 5 animals. Hemodynamic indices and plasma levels of NOS and cGMP were measured. Results Hemodynamics of all 25 rats received operation was poorer compared with the rats of sham group (P<0.01). Hemodynamics of HS group and non-HS+L-Arg group was improved compared with control group (P<0.01), which that of HS+L-NAME was not improved. The plasma levels of NOS and cGMP in the animals received operation were higher than those in the animals of sham group (P<0.01). The plasma levels of NOS and cGMP in HS group were higher than those in control group and before heat stress(P<0.01), and in non-HS+L-Arg group were also higher than those in control group. Conclusion Heat stress in animals with chronic heart failure can protecte myocardium and improve hemodynamics, which was related to heat stress-activated NOS-cGMP.
