盲肠结扎穿刺致豚鼠急性肺损伤相关指标的检测

摘　　要：目的通过对豚鼠(CLP)盲肠结扎穿刺进行不同时间指标的检测,判断急性肺损伤的发生。方法实验中通过不同时间点对动脉血气分析、外周血白细胞计数、肺湿重/干重比值(W/D)及肺组织病理观察对模型进行评价。结果实验组动物在6h后开始缓慢出现症状,呼吸急促100～110次/分(正常呼吸80～95次/分),蜷缩,对外界刺激敏感;16h有少量泡沫状分泌物由鼻腔溢出,呼吸变得窘迫,24h呼吸窘迫十分显著,泡沫状分泌物(带血性)由鼻腔溢出明显增多,呼吸频率为100～140次/分;24h内死亡率可达10%,24～40h可达30%,40～56h可达80%。动脉血Pa02在12h开始明显下降,24h以后Pa02持续低于10kPa,外周血白细胞计数从12h开始降低,24h明显降低,对照组则无明显改变。结论用盲肠结扎穿刺的方法,在术后24h可形成较为典型的、表现更类似于人类的肠源性肺损伤,且症状缓慢发生,逐渐恶化,有利于观察和进行各种干预。Objective To understand occuring acute lung injury (ALI)in guinea pigs through detecting targets at 6h, 16h, 24h after the ligated and perforated cecums of guinea pigs. Methods In ″CLP″ animal model, the cecums of guinea pigs were ligated and perforated and in non-″CLP″. Artery blood gases, peripheral blood leucocytes count, wet/dry weigh ratio of the lung were determined respectively, and pathologic morphometry of the lung were observed for evaluating the ″CLP″ animal model. Results In ″CLP″ animal model, trachypnea, crispation, irritation occurred gradually after 6h, a small number of foam sputum overflow, distress, lassitude, less activity were observed after 16h and distress was very markable, foam secretions (bloody sputum) overflowed from nose after 24h. The rates of respiration was from 100 to 141 times/min. The mortality was 10% within 24h, with 30% within 24h～40h and 80% within 40h～56h. Artery blood oxygen pressure (PaO2) obviously decreased after 12h, being deteriorated until PaO2 consistantly under 10 Kpa after 24h. Peripheral blood leucocytes count began to decrease after and obviously decreased after 24h. Conclusions The manifestation of CLP model seems more similar to septic lung injury caused by intestinal bacteria translocation in human, these symptom occurred gradually, deteriorated stepedly so that it is beneficial to be observed and interrupted.
