心脏不停跳心内直视手术的临床研究

摘　　要：目的　探讨心脏不停跳心内直视手术的方法和意义。方法　随机选择右心系统的先心病病人 ,各 19例给予浅低温和常规体外循环下心内直视手术 ,对术中、术后的各项指标进行总结分析。结果　全组无手术死亡 ,不停跳组手术时间短 ,术后血流动力学平稳 ,血管活性药物用量少 ,心律失常、心肌酶改变等明显优于停跳组 (P <0 0 1)。结论　该手术时间短 ,心肌的损伤及术后并发症少 ,是先心病的最佳治疗方法之一。Objective To evaluate the clinical effects of open heart surgery with beating heart.Methods From 1998 to 2003,38 patients were randomly divided into two groups.19 patients received open heart surgery under cardiopulmonary bypass(CPB),and other received open heart surgery and CPB with a beating heart,and were compared.Results Beating heart group pump stopped immediately at the end of operation.The patients only needed low dose of relevant vasodilators to support circulation after operation,and the study group did better than the control group in terms of serious arhythmia and myocardial enzyme change.Conclusion Open heart surgery with beating heart lasts for shorter time,incurrs less myocardial rujury,and reduces complications after operation.
