血清前S1蛋白在乙型肝炎病毒感染中的临床意义

摘　　要：目的　分析前S1蛋白与其他HBV检测指标的关系 ,并探讨其相应的临床意义。方法　采用ELISA方法对住院患者进行血清前S1蛋白检测 ,比较不同HBV指标组合模式中的前S1蛋白变化。结果　HBV高复制组 (HBeAg、HBVDNA、HBcAbIgM阳性各组 )中血清前S1蛋白阳性高 ,低复制组相对低。HBsAg、HBsAb阳性 (1 8)及HBsAg、HBsAb、HBeAb阳性 (7)和仅HBsAg阳性 (2 1 )患者中 ,其前S1蛋白均阴性。前S1蛋白阳性亦见于HBsAb阳性的大三阳 (3/ 1 2 )、HBsAb阳性的小三阳 (5 / 2 0 )、HBsAg、HBsAb、HBcAb阳性 (1 / 3)和仅HBsAb阳性 (1 / 4 39)及仅HBcAb阳性(1 / 3)患者中。结论　血清前S1蛋白不仅是一反映HBV复制状态和传染性的指标 。Objective To observe changes of serum PreS1 protein in different kinds of HBV marker combination model in patients with liver disease and to explore its clinical significance.Methods To examine serum PreS1 protein with ELISA technique in different kinds of HBV marker combination model in patients with liver diseases.Results In different kinds of HBV marker combination model in patients with active viral replication(such as HBeAg positive,HBV DNA positive,HBcAb IgM positive),the positive rates of preS1 protein were higher than those in patients with inactive viral replication(Chi Square=31.7614,P value<0.0001).And in all patiens of HBsAg,HBsAb positive(18),and HBsAg,HBsAb,HBeAb positive(7)and only HBsAg positive(21),PreS1 protein was negative.And in patiens of HBsAg,HBsAb,HBeAg and HBcAb positive(3/12),HBsAg,HBsAb,and HBcAb positive (5/20),HBsAg,HBsAb and HBcAb positive(1/3),only HBsAb positive(1/439) and only HBcAb positive(1/3),PreS1 protein was positive.Conclusion Serum PreS1 protein is not only a marker of HBV replication and infectivity,but also an indication of HBV particles state and occult HBV infection.
