血浆脂联素和肿瘤坏死因子-α在2型糖尿病合并冠心病患者中的作用

摘　　要：目的 :探讨血浆脂联素 (adiponectin)和肿瘤坏死因子 α(TNF α)在 2型糖尿病合并冠心病病人中的作用。方法 :对 86例 2型糖尿病人 (其中冠心病人 5 3例 ,单纯糖尿病人 3 3例 )进行空腹血糖 (FPG)、血总胆固醇(TCH )、甘油三酯 (TG)、体重指数 (BMI)、胰岛素敏感指数 (ISI)、空腹胰岛素 (FINS)、血浆脂联素和TNF α的检测 ,并与对照组比较。结果 :2型糖尿病合并冠心病组FPG、FINS、TCH和TG明显高于单纯糖尿病组 (P <0 0 5 ) ,BMI与单纯糖尿病组无明显差异 (P >0 0 5 ) ,ISI明显低于单纯糖尿病组 (P <0 0 5 ) ,血浆TNF α浓度明显高于单纯糖尿病组 (P <0 0 1) ,血浆脂联素水平明显低于单纯糖尿病组 (P <0 0 1)。血浆脂联素水平与胰岛素敏感指数呈显著正相关 (P <0 0 5 ) ,血浆TNF α水平与胰岛素敏感指数呈显著负相关 (P <0 0 5 )。结论 :血浆脂联素在 2型糖尿病合并冠心病的发生中发挥保护性作用 ,而TNFObjective To investigate the role of plasma adiponectin and tumor necrosis factor-α (TNF-α) in the type 2 diabetes mellitus patients with coronary heart disease. Methods Compared with those in the control group, the parameters of fasting plasma glucose (FPG), fasting insulin level in plasma (FINS), total cholesterol (TCH) and triglyceride (TG), insulin sensitive index (ISI), body mass index (BMI), plasma adiponectin and tumor necrosis factor-α were measured in 86 patients with type 2 diabetes mellitus (in which 53 patients with coronary heart disease and 33 patients without coronary heart disease). Results Compared to that in the type 2 diabetes mellitus patients without coronary heart disease, the level of FPG, FINS, TCH and TG in type 2 diabetes mellitus patients with coronary heart disease were significantly higher (P<0.05) and the concentration of plasma adiponectin is significantly lower (P<0.01), and the level of TNF-α were significantly higher (P<0.01). The concentration of plasma adiponectin is positively related to the ISI and the level of TNF-α is negatively to the ISI. Conclusion Plasma adiponectin is a role of protection and TNF-α is a role of impairment in the pathogenesis of type 2 diabetes mellitus patients with coronary heart disease.
