摘  要:目的 :评价多种方法检测幽门螺杆菌 ( H.pylori)感染的准确性。方法 :对 82例因上消化道症状接受胃镜检查患者应用酶免疫反应试验原理进行粪抗原 ( Hp SA)试验 ,同步进行胃粘膜快速尿素酶试验 ( RUT)和组织学 ( MB法 )检测 ,其中 67例患者进行 14 C-呼气试验 ( 14 C- UBT)检测H.pylori,以快速尿素酶试验和组织学联合检测作为金标准。结果 :Hp SA检测诊断 Hpylori感染的敏感性 93.30 % ,特异性 92 .5 0 % ,阳性预测值 90 .30 % ,阴性预测值 94.87% ,准确性 92 .90 % ;14 C-UBT检测诊断 H.pylori感染的敏感性 81 .81 % ,特异性 81 .0 8% ,阳性预测值 72 .0 0 % ,阴性预测值 88.2 4 % ,准确性 81 .36%。结论 :Hp SA试验、14 C- UBT试验是诊断 H.pylori感染的简便。Objective: To evaluate the accuracy of muti tests methods for detecting on Helicobactor Pylofis. Methods: 88 patients underwent endoscopy who suffered from upper gastrointenstinal symptom. HpSA was determined by enzyme immunoassay.The status of H.pylori infection was determined in all patients by stool HpSA test,rapid urease test (RUT) and histology simultaneously. 67 cases among 88 cases were determined by 14 C urea breath test( 14 C UBT). It was used as the gold standard to combining RUT AND histology determination.Results: The sensitivity of HpSP test for the diagnosis of H.pylori infection was 93.3%(28/30) and specificity was 92.50%(51/40). The positive predicitive values and negative predicifive values were 90.30%(28/31) and 94.87%(37/39) respectively. The total accuracy was 92.90%(65/70). The sensitivity of 14 C UBT foe the diagnosis of H.pylori infection was 81.8%(t8/22) and its specificity was 81.08%(30/37). The positive predictive values and negative predictive values were 72.00%(18/25) and 88,24%(30/34) respectively, The total accuracy were 81.36%(48/59). Conclusion:HpSA and 14 C UBT are two sorts of simple and convenient, noninvasive and highly accurate methods in the diagnosis of H.pylori infection.
