摘 要:目的探讨预防血液透析中低血压反应的方法。方法对血透中经常出现低血压反应的规律血液透析患者10例,采用单盲法,自身对照设计。常规透析治疗2周后,再分别接受钠制式血液透析、序贯血液透析、低温血液透析3种治疗方案,每种方案为期2周。结果钠制式血液透析、低温血液透析与常规透析相比,低血压发生率以及护理干预次数明显降低,序贯血液透析组降低则不明显。结论对血液透析过程中有低血压倾向的患者,可选用低温血液透析或钠制式血液透析,序贯血液透析过程中低血压发生率较高。Objective To explore the methods to prevent intradialytic hypotension at hemodialysis. Methods A single-blinded, crossover study design of four different protocols was undertaken in 10 hemodialysis patients with a prior history of intradialytic hypotension. Each patient undergoing standard hemodialysis for two weeks was subjected to another three test protocols such as sodium modeling dialysis, ultrafiltration followed by dialysis and low temperature dialysis, each for two weeks. Results There were significantly fewer hypotensive episodes and nursing interventions in the sodium modeling and low temperature protocols as compared with the standard one. Ultrafiltration followed by dialysis was associated with a significantly greater number of hypotensive episodes and nursing interventions than any of the two test protocols. Conclusion In combating intradialytic hypotension, sodium modeling and low temperature proctocols are more effective than ultrafiltration dialysis.
