Biomechanical Effects of the Coralline Vertebra Interbody Fuser after Resection of Nucleus Pulposus of Intervertebral Disc

摘  要:目的通过动物实验观察髓核切除前后腰椎间盘承受轴向载荷的形变与植入人工珊瑚椎间融合器后对形变的影响。方法采用20个猪的L3,4、L4,5椎体及所属的椎间盘作为力学实验模型,轴向加压分别测试不同载荷下椎间隙高度的变化,椎间盘髓核摘除前、后及植入人工珊瑚椎间融合器后分别测试3组数据,资料采用Paired-Samplesttest统计分析。结果椎间盘切除术后椎体前柱的稳定性破坏,较小的压力引起较大的椎间隙高度变化(P<0.001),将椎间融合器植入椎间隙后,脊柱前柱的稳定性获得恢复,由轴向压力引起的前柱不稳定现象消失,椎体间隙的高度变化小(P>0.1)。结论初步结果表明,人工珊瑚椎间融合器植入摘除髓核的椎间隙,将有效地防止术后椎间隙狭窄,改善脊柱的生物力学特性。但仍需做更大规模实验进一步研究。Objective To study the effects of axial compression load on the shape changes of the lumbar vertebral disc after resection of nucleus pulposus of intervertebral disc and coralline vertebra interbody fuser implantation in the gap of lumbar vertebral disc. Methods Twenty cases of porcine L3,4 and L4,5 vertebra body with its vertebral disc were enrolled as biomechanical test model.The changes of intervertebral width under axial compression with different loads were measured in three conditions--pre and post resection of nucleus pulposus of intervertebral disc,and implantation of coralline vertebra interbody fuser.The resulted data were statistical analyzed by the Paired-Samples t test. Results Smaller axial pressure would induce larger changes of intervertebral width(P<0.001?雪.It demonstrated that after the resection of neucleus pulposus of intervertebral disc,the stability of anterior spinal column was damaged. But coralline vertebra interbody fuser implantation would restore the stability of the anterior spinal column and the axial pressure induced the unstability of the anterior spinal column disappeared and little chang of the intervertebral width was found(P>0.1).Conclusion The preliminary results demonstrate that implantation of coralline vertebra interbody fuser can prevent the narrowing of the intervertabral space after resection of nucleus pulplosus of intervertebral disc and improve the biomechanical characteristic of spinal column.The larger scale experiments should be performed for further study.
