80岁以上高龄股骨颈骨折关节置换术手术安全性的研究

摘　　要：目的　研究人工关节置换术治疗高龄股骨颈骨折围手术期安全性 ,以提高高龄及超高龄者的生活质量。方法　对高龄股骨颈骨折进行关节置换术 78例 ,男 33例 ,女 4 5例。年龄 80～ 89岁 5 3例 ,90～ 99岁 2 4例 ,1 0 0岁以上 1例。全髋关节置换术 2 3例 ,人工股骨头置换术 5 5例。骨折前有并存症 72例 ,其中二种并存症 4 9例 ,三种 2 9例 ,四种及以上并存症 1 6例。结果　术后发生并发症 2 7例 ,发生率为 34 6 % ,死亡 3例 ,按照Harris功能评分 ,优 2 6例 ,良 1 8例 ,优良率为 78 6 % ,可 7例 ,差 5例。结论　人工关节置换术是治疗高龄股骨颈骨折的有效方法 ,但应注意病例的选择及并存症的处理和并发症的防治。Objective To study safety of hip joint replacement on the femoral neck fractures in the advanced age to enhance life quality.Methods Seventy-eight cases of the femoral neck fractures with sub-head type,via-neck type and Garden Ⅲ,Ⅳ type were treated with total hip replacement or femoral head replacement.Seventy eight cases were involved,male 33 cases,female 45 cases.Their age was 80～101 years old, 53 cases 80-89,24 cases 90～99,1 case over 100. Twenty-three cases were treated with total hip replacement,the rest were treated with femoral head replacement.Before fracture,72 cases had the other disorders.Two coexist disorder were in 49 cases,3 coexist disorders 29, over 4 coexist disorder 16. Results After operation,complications were occurred in 27 cases, account for 34 6% , death 3 cases.According for Harris function score 26 cases were excellent cases,18 good,7 general, 5 bad.Conclusion Hip joint replacement is effective method to the femoral neck fractures in the advanced age, but it is paid attention to choice of operative case, treatment of complication and coexist disorders.
