手部骨关节皮肤缺损同种异体骨关节移植皮瓣修复

摘　　要：目的　探讨手部骨关节皮肤缺损同种异体骨关节移植皮瓣修复的临床效果。方法　采用同种异体掌指、指间关节移植 ,3种逆行岛状皮瓣修复皮肤缺损 ,根据需要可携带异体肌腱、关节囊。结果　治疗全掌指关节缺损 5例 ,全近指间关节缺损 2例 ,单纯掌骨头缺损 3例 ,单纯近节指骨头缺损 1例 ,获得成功。修复后的骨关节对位好 ,关节间隙存在 ,骨愈合良好 ,术后 5个月关节活动良好 ,皮瓣全部成活。结论　术式简单实用 ,效果可靠 ,损伤小 ,骨愈合率高 ,可同时利用异体骨关节携带的肌腱、关节囊一期修复肌腱。Objective To investigate the clinical effects of repair the defects in joint and skin of hand by allograft of compound joint skin flaps.Methods Adopt Allograft metacarpophalangeal and interphalangeal articulations transplantation ,three kinds of reverse island skin flaps to repair composite tissue defects in joint and skin.Allograft tendon and joint capsule can be taken,according to the requirement.Results All flaps survived.The defects of joints include metacarpophalangeal in 5 cases, interphalangeal in 2cases,top of metacarpal bone 3 case and top of phalanxin in 1 case were cured. The joints healed in a good state. The joints acted almost normallay 5 months later.Conclusions The operation is simple and useful with reliable effects. The composite defects of tendon and joint capsule or multicipal joints can be reconstructed by autogenous tendon or allograft of joint accomplished allograft tendon.
