冠状动脉病变程度与左室舒张功能的关系

摘　　要：目的　探讨冠状动脉病变程度与左室舒张功能之间的关系 ,以及年龄对左室舒张功能的影响。方法　10 6例进行选择性冠状动脉造影的住院患者按冠状动脉病变程度分为冠状动脉正常年龄 <5 0岁组、冠状动脉正常年龄≥ 5 0岁组、单支病变组、双支病变组、三支病变组 5组 ,测定和比较左室舒张功能指标E峰值、A峰值、E/A比值。结果　单支病变、双支病变组E峰、E/A比值下降 ,三支病变组E峰、E/A值无改变 ,冠状动脉正常年龄≥ 5 0岁组E峰、E/A值较 <5 0岁组明显减低、A峰值明显升高。结论　冠心病早期左室舒张功能下降 ,随病变程度加重 ,左室舒张功能呈“伪正常”化 ;年龄对舒张功能呈增龄性影响。Objective To investigate the relationship of coronary arteriopathy severity with left ventricular diastolic function and the patients' age.Methods One hundred and six patients undergoing a selective coronary angiography were divided into five groups according to coronary arteriopathy severity,such as younger than 50 years old with normal coronary artery group, over 50 years old with normal coronary artery group,mono-branch coronary arteriopathy group,bi-branch coronary arteriopathy group,tri-branch coronary arteriopathy group.The peak values of E, A and E/A ratio were detected and compared.Results The peak values of E and E/A ratio in the groups of mono-branch coronary arteriopathy and bi-branch coronary arteriopathy decreased.The peak values of E and E/A ratio in the tri-branch coronary arteriopathy group did not change.The peak values of E and E/A ratio in over 50 years old with normal coronary artery group were significantly decreased compared with those in younger than 50 years old group.Conclusion In the early stage of the coronary disease,the left ventricular diastolic function began to decrease.With more severity of the coronary arteriopathy the left ventriculer diastolic funtion appeared to false normal.Age was positively associated with left ventricular diastolic function.
