白叶柴虎汤抑制乙肝病毒(HBV)复制的临床研究

摘　　要：以抑制HBV复制与提高机体免疫功能为原则 ,以扶正祛邪为治则 ,以疏肝健脾养阴、活血、清热、解毒及利湿为治法 ,并且适合于中医辨证四型乙肝服用的原则 ,自拟方剂———白叶柴虎汤 ,对HBV有明显的抑制作用。目的 :探讨白叶柴虎汤对乙肝病毒 (HBV)复制的抑制作用。方法 :根据清热解毒、健脾养肝法 ,自拟“白叶柴虎汤”对HBV复制的抑制作用进行临床研究 ,并设干扰素、拉米夫定及常规护肝组以对照疗效。结果 :1)白叶柴虎汤组HBeAg、HBVDNA转阴率及ALT复常率分别为 5 7.0 %、43 .1%及 80 .0 % ,干扰素组HBeAg、HBVDNA转阴率及ALT复常率分别为 5 0 .8%、47.6%及 77.7% ,拉米夫定组HBeAg、HBVDNA转阴率及ALT复常率分别为 10 .0 %、69.7%及 81.8% ,常规护肝组HBeAg、HBVDNA转阴率及ALT复常率分别为 5 .0 %、11.1%及 74.1% ,经统计学处理发现 :白叶柴虎汤组ALT复常率较其它组比较无显著性差异 ,HBeAg、HBVDNA转阴率较常规护肝组明显提高 (P <0 .0 0 1) ,但与干扰素组比较无显著性差异 ;HBeAg转阴率较拉米夫定组高 ,但HBVDNA转阴率较拉米夫定组低。 2 )白叶柴虎汤组各证型的HBeAg、HBVDNA转阴率及ALT复常率均无显著性差异。结论 :白叶柴虎汤对HBV有一定的抑制作用 ,并可作为治疗慢性乙型肝炎的专病专方。Objective:To study the effect of the specical traditional Chinese medicine Baiye Chaihu Solution on hepatitis B infection.Method:We desiged the prescription-'Baiye Chaihu Solution'according to its principle of invigorating spleen,hourishing liver cleaning away heat-evil and promoting diuresis.We observed the serum HBeAg?HBVDNA negative transformation and ALT normalization in 'Baiye Chaihu Solution' Group? interferon(IFN)group?lamivudine group and liver-aid group.Results:The ratio of HBeAg?HBVDNA negative transformation and ALT normalization in 'Baiye Chaihu Solution' Group are 57.0%?43.1%?80% respectively,in IFN group 50.8%?47.6%?77.7%,in lamivudine group 10.0%? 69.7%?81.8% respectively in liver-aid group 5.0%?11.1%?74.1% respectively No significant difference exists in 'Baiye Chaihu Solution'group compared with other groups on the ration of ALT normalization.No significant difference exists between 'Baiye Chaihu Solution'group and IFN group on the ratio of HBeAg? HBVDNA negative transformation,but the ratio are higher in the Baiye Chaihu Solution'group than in the liver-aid group.The ratio of HBeAg negative transformation are higher in the Baiye Chaihu Solution group than in the lamivudiine group.but the ratio of HBVDNA negative transformation are lower in the Baiye Chaihu Solution group than in the lamivudine.2) No significant difference exists among the subgroups of different symptoms.Conclusion:Baiye Chaihu Solution can be recommended as a special prescription for chronic hepation B infection.
