摘  要:目的 探讨高血压患者 (EH)基础及急性糖负荷后内皮依赖性血管舒张功能 (EDF)的变化。 方法 2 6例 EH (EH组 )和 1 5例对照者 (对照组 )禁食 1 2 h后口服葡萄糖 75 g,服糖前及服糖后 1 ,2 ,3h采用彩超检测EDF,分别测定血清抗氧化产物 (SOD、T- AOC、Anti O2 - )、血糖、胰岛素及空腹血脂。 结果  EH组的基础 EDF较对照组显著降低 [(1 3.6 4± 5 .0 1 ) % vs (1 7.4 3± 5 .6 6 ) % ,P<0 .0 5 ]。糖负荷后 1 h,两组 EDF较各自基础值显著降低 [(9.4 8± 3.33) % vs (1 3.6 4± 5 .0 1 ) % ;(1 4 .35± 5 .86 ) % vs (1 7.4 3± 5 .6 6 ) % ,P<0 .0 5 ];且 EH组较对照组降低程度更为明显 [(31 .74± 6 .91 ) % vs (1 3.0 3± 5 .94 ) % ,P<0 .0 5 ]。 结论  EH基础内皮功能较正常者降低。在急性糖负荷后 ,高血压及正常组内皮功能均明显受损 。Objective To explore the change of endothelial dependent flow mediated vasodilatation after acute glucose loading in essential hypertension. Methods Twenty six patients with essential hypertension and fifteen healthy controls were studied. The acute effects of oral glucose loading(75 g) on endothelium dependent flow mediated dilation(EDF) of the brachial artery was assessed by high resolution ultrasound technique on 0, 1, 2, 3 hour after glucose loading respectively. At the same time. The concentrations of blood sugar, serum insulin, superoxide dismutse(SOD), total antioxidative capacity(TAOC) and anti superoxide anion(Anti O 2 -) were measured. Results The baseline EDF in patients with EH was significantly lower compared with the control subjects[(13 64±5 01)% vs(17 43±5 66)%,P<0 05]. EDF in both EH and control subjects was impaired significantly[(9 48±3 33)% vs(13 64±5 01)%, (14 35±5 86)% vs(17 43±5 66)%,P<0 05] 1 hour after acute glucose loading; but this impairment in EH was more serious than that in control subjects[(31 74±6 91)% vs(13 03±5 94)%,P<0 05]. Conclusion The baseline EDF in patients with EH is significantly lower than that in control subjects. EDF in both of patients with EH and control subjects is impaired after oral glucose loading, but the impairment is more serious in EH than that in control subjects.
