VWF、D-D、ATⅢ联合测定与冠心病关系探讨

摘　　要：目的　探讨冠心病患者血浆中几种凝血活化分子标志物变化的关系。方法　将 334例冠心病住院患者的血浆用法国STAGO公司生产的STAGOSTA全自动血凝仪分别测定血管性血友病因子 (VWF)、D二聚体 (D D)、抗凝血酶Ⅲ (ATⅢ )含量。结果　 334例患者VWF的平均含量在16 9 35 % ,明显高于参考值上限范围结果 (5 0 %～ 16 0 % ,平均 10 5 % ) ,差异有显著性 (P <0 0 1) ;D D平均值达 710 μg/L也高于本室上限 (0～ 5 0 0 μg/L ,平均 2 5 0 μg/L) ,差异有显著性 (P <0 0 1) ;而ATⅢ虽然离散度较大 (最低 2 2 % ,最高 16 6 % ,平均 10 2 2 7% ) ,与本室的 80 %～ 12 0 %参考范围大致相似 ,但统计学分析 ,差异无显著性 (P >0 0 5 )。结论　VWF及D D测定对冠心病的病情观察意义较大 ,而ATⅢ测定对冠心病病情观察意义不大。Objective To investigate relationship between changes of several plasma coagulation activation molecular markers in patients with coronary heart disease (CHD). Methods The plasma levels of von willebrand factor(VWF),D-dimer(D-D) and antithrombin Ⅲ (ATⅢ) were measured from 334 cases of CHD, respectively . All sample were detected by STAGO STA automaticcoagulationmeter from France. Results The mean content of plasma VWF and D-D in patients with CHD was significantly higher than that of reference value in our laboratory(169.35%vs 105%,P<0.01 and 710 μg/L,vs 250 μg/L,P<0.01 respectively). However,plasma ATⅢ had no significant difference between CHD patients and the nomal reference value (102.27%vs 100%,P>0.05) though its vicariance degree was notable. Conclusion The detection of VWF and D-D can be used to monitor the state of CHD, while the degree of ATⅢ has little value.
