摘 要:目的 探讨声学造影后肝实质灰度时间 强度曲线 (time intensitycurve ,TIC)在诊断实验性肝硬化中的应用价值。方法 通过皮下注射四氯化碳复制大鼠肝硬化模型 ,经外周静脉声学造影后应用视频分析法形成肝脏实质灰度的时间 强度曲线 ,测量并分析峰值时间 (timetopeak ,TP)、峰值强度 (peakintensity ,PI)、峰值减半时间 (halftimeofdescent ,HT)、曲线下降斜率 (descendingslope ,DS)、曲线下面积 (areaunderthecurve ,AUC)和平均渡越时间 (meantransittime ,MTT)等参数。结果 肝硬化组大鼠的TP、PI、DS和AUC测值明显低于正常对照组 (P <0 .0 1) ,HT与MTT测值则明显升高 (P <0 0 1)。结论 声学造影后肝实质时间 强度曲线可以反映肝硬化时大鼠肝脏微循环与血流动力学改变 ,有望成为诊断肝硬化的新方法。Objective To discuss the diagnostic value of time intensity curve (TIC) of liver parenchyma with intravenous contrast ultrasound in liver cirrhosis. Methods After the model of liver cirrhosis was established in 16 rats by subcutaneous injection of CCl 4, time intensity curve was drawn by video frequency analysis after intravenous contrast ultrasound and the time to peak (TP), peak intensity (PI), half time of descent (HT), descending slope (DS), area under the curve (AUC), and mean transit time (MTT) were measured and compared. Results TP, PI, HT, and AUC in liver cirrhosis were shorter than those in the normal control significantly ( P <0.01), while DS and MTT were longer ( P <0.01). Conclusion TIC of liver parenchyma after contrast ultrasound can reflect the changes of hepatic microcirculation and hemodynamics in liver cirrhosis, which is hopeful to be a new diagnostic method.
