中重度卵巢过度刺激综合征继发妊娠的妊娠结局分析

摘　　要：目的 探讨应用促排卵技术致中重度卵巢过度刺激综合征继发妊娠与自然受孕妊结局的差异。方法 选择应用促排卵技术致中重度卵巢过度刺激综合征并继发妊娠72例(OHSS组)和同期自然受孕在本院住院分娩的总体(自然受孕组)作比较,比较两组孕妇一般情况、妊娠合并症、分娩情况和新生预后等方面的差异。结果(1)OHSS组孕妇平均年龄(31.2±3.7)岁,自然受孕组孕妇为(27.1±3.7)岁,两组比较,差异有显著性(P<0.05)。(2)OHSS组早期先兆流产17例(23.6%),自然受孕组325例(6.9%),两组比较差异有极显著性(P<0.01);OHSS组患妊娠期糖尿病或糖耐量异常13例(18.1%),自然受孕组120例(2.6%),两者比较差异有极显著性(P<0.01)。(3)OHSS组剖宫产率62.5%(45/72),明显高于自然受孕组48.7%(2286/4695)。(4)OHSS组双胎妊娠8例(11.1%),自然受孕双胎妊娠29例,发生率0.62%,两者比较差异有极显著性(P〈0.01〉。结论 OHSS组的孕妇年龄较大,早期先兆流产、双胎妊娠、妊娠期糖尿病及糖耐量异常的发生率高;分娩方式以剖宫产为主;OHSS孕妇的围产儿结局与自然受孕的相似。Objectives A study on the differences between pregnancy outcomes of spontaneouspregnancy and severe ovarian hyperstimulation syndrome(OHSS)complicated pregnancy after assisted repro-ductive techniques .Methods 72cases sever OHSS complicated pregnancy after assisted reproductivetechniques (OHSS group )were compared with total spontaneous pregnancy patients during the same period(control group)on general conditions ,complications of pregnancy, delivery and neonatal prognosis, etc.Results (1)The mean age :OHSS group (31.2±3.7)years is higher than control group (27.1±3.5)years(P<0.05).(2)Threatened abortion happened on early pregnancy is significantly higher in OHSS group(17cases ,23.6%)than control group (325cases,6.9%)(P<0.01);Gestational diabetes mellitus or gesta-tional impaired glucose tolerance are also significantly higher in OHSS group(12 cases ,18.1%)thancontrol group (120 cases ,2.6%)(P<0.01).3)Caesarian section rate of OHSS group (62.5%)is higher thancontrol group (48.7%,2286/4695).4)Twin pregnancy rate in OHSS group (8 cases ,11.1%)is higher thancontrol group(29 case, 0.062%)(P<0.01).Conclusion OHSS group is predisposed to happen in older age,threatened abortion happened on early pregnancy 、twin pregnancy 、gestational diabetes mellitus orgestational impaired glucose tolerance are high rate of caesarean section .The outcomes of prenatalfetus is the same with spontaneous pregnancy .
