心力衰竭患者外周血粒细胞巨噬细胞集落刺激因子与白细胞介素8的变化及其意义

摘　　要：目的　检测心力衰竭患者外周血粒细胞 巨噬细胞集落刺激因子 (GM CSF)、白细胞介素 8(IL 8)及白细胞的水平 ,并探讨他们在心力衰竭中的关系。方法　 5 9例心力衰竭患者 ,根据NYHA分级分成心功能Ⅱ、Ⅲ、Ⅳ级 3组 ,测定血清GM CSF及IL 8水平采用放射免疫法 ,并镜检血WBC数。并与年龄、性别相匹配的正常对照组 (19例 )进行比较。结果　心力衰竭组外周血GM CSF、IL 8及WBC水平与正常对照组比较有显著性差异 (P <0 .0 1) ,且随心力衰竭程度的加重而更明显 ;不同病因的心力衰竭患者之间无显著差异 (P >0 .0 5 ) ;心力衰竭患者外周血GM CSF与IL 8及WBC呈显著正相关 (r值分别为 0 .6 0 2 ,0 .5 4 1;P <0 .0 1)。结论　心力衰竭患者外周血GM CSF、IL 8及WBC水平增高 ,并随心衰程度的加重而加重 ;提示炎症反应可能参与了心力衰竭的发生和发展 ,而GM CSF在其中可能起重要作用。Objective To investigate the level of serum granulocyte-macrophage colony-stimulating factor(GM-CSF),interleukin-8(IL-8) and WBC in patients with heart failure(HF) and their relationship.Methods GM-CSF, IL-8 and WBC were measured in 59 patients with various degrees of HF and 19 normal control.The patients were divided into 3 groups(cardiac function class II,III,IV,according to NYHA classification),serum GM-CSF and IL-8 were measured by radio-immunology assay.Results As compared with control group,the levels of GM-CSF,IL-8 and WBC were significantly elevated in patients with HF(P<0.01).GM-CSF,IL-8 and WBC were not significant different among various HF groups(P>0.05).GM-CSF was positively related with IL-8 and WBC( r=0.602,0.541,P<0.01).Conclusion The levels of serum GM-CSF,IL-8 and WBC are elevated in patients with HF,and are strongly associated with the degree of cardiac dysfunction,which probably take part in the incidence and development of HF.GM-CSF may play an important role in the pathophysiological development of HF.
