清肝泻心滋阴润燥法改善2型糖尿病胰岛素抵抗的研究

摘　　要：目的 :探讨清肝泻心法、滋阴润燥法及两法合用对 2型糖尿病的影响 ,分别评价其对胰岛素抵抗的疗效的差异。方法 :10 0例病人随机分为上述 3组及西药达美康对照组 ,上述 3组均以达美康作为基础治疗 ,每组各 2 5例 ,疗程均为 4周 ,4组均测服药前后的空腹血糖 (FBG)、空腹胰岛素 (FINS)、计算胰岛素敏感性指数 (IAI)。结果 :(1)联合中药治疗组的降糖、提高IAI显著 (P <0 .0 1) ,但清肝泻心、滋阴润燥法组的效果更显著 (P <0 .0 0 1)。 (2 )清肝泻心、滋阴润燥法的改善胰岛素抵抗的疗效优于其它方法。结论 :清肝泻心、滋阴润燥法治疗 2型糖尿病可能通过降低高胰岛素血症、增强机体对胰岛素的敏感性的胰外作用途径 ,达到减轻外周胰岛素抵抗 ,从而降低血糖的目的。Purpose:To explore the clinical effects of Qing Gan Xie Xin(QGXX)method and Zi Yin Run Zao (ZYRZ)method and that of both of them when being used together in treating type 2 diabetes mellius ,at the same time to evaluate differences of them of the improving insulin resistance one by one.Methods:100 patients with type 2 diabetes are randomly divided into four groups with 25 patients each.They are with methods of QGXX ZYRZ(1 group)?QGXX(2 group)?ZYRZ(3group) and Diamicron (4group).Pre-and Post-treatment for 4 weeks,the level of FBG?FINS?IAI are examined in all the patients.Results:(1) 1?2?3groups can significantly recluce blood sugar and improve IAI.but the QGXX ZYRI group is more is more significant(P<0.01)(2) The effective rate of improving insulin resistance is more significant than other methods(P<0.001)Conclusions:It suggested that the treatment of QGXX ZYRZ method might decrease insulin peripheral resistance and reduce blood sugar by means of reduching hyperinsulinemia and improving insulin sensitivity.
