手部无人区肌腱一期修复后早期活动对手功能的影响

摘  要:目的 通过观察手部无人区肌腱损伤患者一期修复后手功能恢复状况,探讨早期系统康复治疗的合适时间和方法。方法 将58例(108指)各种原因引起的手部无人区肌腱损伤进行一期修复,并在48~72 h后开始运用Kleinert氏手功能支具控制性训练,辅助各种物理治疗,部分运用连续被动运动(CPM)机训练4~12周,采用完全主动活动(TAM)系统评价其功能恢复情况。结果 共随访42例(85指)患者,随访时间5月~3.8年。TAM系统评价获得优22例、良8例、可7例、差5例,优良率为71.43%,有效率为88.09%。结论 手部无人区肌腱损伤经一期修复,早期系统康复训练可以促进手功能恢复,大部分患者可获得较满意疗效。Objective To find suitable time and methods of early rehabilitation of tendon the injured at no man area in hands. Methods One hundred and eight injured tendons at no man area of 58 patients were reconstructed in one stage. Controlled training by Kleinert's functional splints was started 48-72 h after operation, assisted by different kinds of physical therapy. A partion of the cases accepted CPM training for 4-12 weeks. Results The rehabilitation result of 42 cases in follow up was evaluated by the total active motion system. Twenty-two patients got excellent result, 8 good, 7 ordinary and 5 bad with excellent rate 71. 43% and efficient rate 88. 09% . Conclusion After one-stage reconstruction operation of the hand tendons at no man area, early systemic rehabilitation training is helpful for hand function recovery.
