腓骨滋养动脉及其临床意义

摘　　要：目的 为带血管游离移植腓骨提供解剖学基础。方法 在69例尸体下肢标本上,对腓动脉起点至腓骨滋养动脉间的距离,腓骨滋养动脉至腓骨小头的距离,腓动脉行程、外径、周围关系以及滋养动脉的长度和外径进行了观察。结果 腓动脉起点至腓骨小头的距离为6.66mm±1.95cm,腓骨滋养动脉起点至腓骨小头的距离为13.20±2.46cm,腓骨脉外径为2.52±0.84mm,腓骨滋养动脉外径为0.79±0.23mm。结论 腓骨中段血供丰富,如果截取的腓骨超过腓骨全长的1/3,宜取中1/3为佳。如果截取较短的腓骨,宜取腓骨中、上段为佳。腓动脉走行紧贴或紧邻腓骨下行,手术中必须加以保护。Objective To afford the anatomic basis for transplantation of isolated fibula with blood vessels. Methods 69 examples of lowerextremity specimens were dissected to measure and observe the distance between the beginning of arteria fibularis and nutrient artery of fibula, the distance between nutrient artery of fibula and capitulum of fibula, the route, external diameter of arteria fibularis and the relations between it and the surroundings, and the length and external diameter of its nutrient artery. Results The distance between the of arteria fibularis and nutrient atrery of fibula was 6.66cm±1.95cm, the distance between the beginning of nutrient artery of fibula and capitulum of fibula was 13.20cm±2.46cm, the external diameter of arteria fibularis was 2.52mm±0.84mm and that of nutrient artery of fibula was 0.79mm±0.23mm. Conclusion The middle segment of fibula is rich in blood supply, if the amputated fibula is above one-third of its whole length, it is suitable to amputate the one-third in the middle of fibula; if the amputated fibula is shorter, it is suitable to amputate its middle and upper segments, in addition, arteria fibularis must be protected in the operation because it runs downward closing or being next to fibula.
