特发性肺纤维化患者血清及支气管肺泡灌洗液中转化生长因子-β和Ⅲ型前胶原含量变化的关系

摘　　要：目的 :研究特发性肺纤维化 (IPF)患者血清及支气管肺泡灌洗液 (BALF)中转化生长因子 - β(TGF - β)含量的变化 ,并以支气管肺泡灌洗液中Ⅲ型前胶原 (PCⅢ )水平作为胶原代谢的标志 ,探讨TGF - β在IPF发病中的作用。方法 :正常对照组 8例 ,IPF组 16例 ,行支气管镜检查后取BALF并同时抽血留取血清 ,采用ELISA法测定BALF中TGF - β、PCⅢ水平及血清中TGF - β水平。 结果 :IPF组BALF中TGF - β水平明显高于对照组 (P <0 .0 1) ;血清中TGF - β水平亦明显高于对照组 (P <0 0 5 ) ;IPF组BALF中PCⅢ的水平明显高于对照组 (P <0 .0 1) ;IPF组BALF中TGF - β水平与PCⅢ水平呈线性正相关 (γ =0 .79,P <0 .0 1) ;IPF组患者BALF中TGF - β水平和血清中TGF - β水平呈线性正相关 (γ =0 .77,P <0 .0 1)。 结论 :IPF患者肺内TGF - β表达水平升高 ,其变化水平与肺内成纤维细胞的活动性及胶原代谢的活动性相关 ,推测TGF -Objective:To determine the level of transforming growth factor-β(TGF-β) in peripheral blood and bronchoalveolar lavage fluid (BALF) of patients with idiopathic pulmonary fibrosis (IPF), and regarded procollagen Ⅲ (PCⅢ) as a marker of college metabolism, to clarify the role of TGF-β in the pathogenesis of IPF.Methods: The bronchoalveolar lavage was performed to all patients with IPF and 8 healthy controls,and serum was collected at the same time. The levels of PCⅢ and TGF-β in the BALF and serum were measured byusing ELISA assays. Results: The levels of TGF-β and PCⅢ in peripheral blood and in BALF were significantly higher in patients with IPF than control subjects (P<0.01). In IPF patients, the BALF concentration of TGF-β was proportionally correlated with concentration of PCⅢ(γ=0.79, P<0.01), and the BALF concentration of TGF-β was proportionally correlated with the serum concentration of TGF-β(γ=0.77, P<0.01). Conclusions: Excessive TGF-β level exists in patient with IPF, which was proportionally correlated with the activity of collagen metabolism, so it may play a relevant role in the pathogenesis of IPF.
