超声评价二尖瓣替换术保留瓣下结构对左心功能的影响

摘  要:目的 用超声心动图评价二尖瓣替换术保留瓣下结构对术后左心功能的影响。方法 将 96例患者分为两组 ,其中 5 6例保留瓣下结构为观察组 ,40例按传统方法手术为对照组。应用 HP-1 0 0 0型多功能心脏超声诊断仪分别在术前和术后 3个月测定 EF、FS、SV、LVEDV、LVESV、LVd、LVs、LAD等心功能多项指标。结果 术前两组间的EF、FS、SV、LVEDV、L VESV、LVd、LVs、LAD均无显著性差异 ( P >0 .0 5 )。术后两组 LAD和 LVd均较术前明显缩小 ( P <0 .0 5 ) ,术后观察组 EF、FS、SV等心功能多项指标明显优于对照组。结论 二尖瓣替换术保留瓣下结构有益于术后左心功能的维护。Objective To evaluate the effect of left ventricular function after mistral valve replacement with preservation of the posterior leaflet by ultrasonography.Methods 96 patients were divided into 2 groups. The patients in experimental group (n=56) underwent mistral valve replacement with preservation of the posterior leaflet and those in control group (n=40) underwent traditional operation. All patients were examined by HP-1000multifunctional cardiac ultrasonic diagnostic apparatus before and after operations. Ejection fraction of left ventricle (EF),fraction shortening of left ventricle (FS),stroke volume (SV),left ventricular end diastolic volume (LVEDV),left ventricular end systolic volume (LVESV),left ventricular end diastolic diameter (LVd),left ventricular end systolic diameter (LVs) and left atrium diameter (LAD) were recorded.Results The values of EF,FS,SV,LVEDV,LVESV,LVd,LVs and LAD were no significan,LVs and LAD were no significanations (P>0.05). The values of LAD and LVd reduced in evidence after operations(P<0.05). The values of EF,FS,and SV of experimental group were better than those of control group.Conclusions Mistral valve replacement with preservation of the posterior leaflet is beneficial to left ventricular function after operation.
