腹腔镜胆囊切除术用小量乌拉地尔控制心血管副反应的临床观察

摘　　要：目的　探讨应用小量乌拉地尔控制腹腔镜胆囊切除术中心血管副反应的临床效果。方法　选择 ASA - 级腹腔镜胆囊切除术病人 3 4例 ,随机分为对照组 1 7例 ,实验组 1 7例 ,静脉注射乌拉地尔 1 0 mg。麻醉插管后行控制呼吸 ,气腹后静脉注射乌拉地尔 1 0 mg,记录充气前、充气后、静脉注射乌拉地尔后 1、3、5、1 0 min时 SBP、DBP、HR数值。结果　实验组在气腹后静脉注射乌拉地尔 1 0 mg,能有效地缓解和控制腹腔镜手术气腹后血压的剧烈变化。与对照组比较 1 min时 SBP下降 1 6.3 % ,DBP下降 1 5 .1 % ,HR无显著变化。用药后各时点血压与对照组比较统计学差异显著 ,以 3～ 5 min时为著 ( P <0 .0 1 )。结论　静脉注射小量乌拉地尔对控制气腹后血压剧增效果确切。Objective To providing a little urapidil to control the clinical cardiovascular side effect in laparoscopic operation of cholecystectomy.Methods 34 patients who had been taken on ASA Ⅰ-Ⅱ grade laparoscopic operation of cholecystectomy,were divided into two groups:control group including 17 samples and experimental group including 17 samples,Experimental group was given intravenous 10 mg of urapidil.Carry out commanding breath after anesthesia intubation,inject intravenously 10 mg of urapidil after perironeal insufflation of CO_2.Observe and write down the datum of SBP,DBP,HR of receiver of before and after of perironeal insuffiation of CO_2,on the side of datum in 1,3,5,10 minutes of apply of urapidil.Results 10 mg of urapidil intravenously can improve and control the radical fluctuate of blood pressure obviously caused by perironeal insuffiation of laparoscopic operation.Compare to the contral group,SBP and DBP of the reducing rate of expefimenml group in 1 minute was 16.3% and 15.1% respectively,whereas the HR have little descent.Obvious variation was observed followed by injection,it's remarkable in 3,5 minutes especially.Conclusions A little urapidil by injected intravenously can control violent heighten effectively followed by perironeal insuffiation of CO_2.
