环孢素A与泼尼松、水飞蓟素联合治疗Graves眼病

摘　　要：目的　探讨运用环孢素A(CsA)与泼尼松、水飞蓟素联合治疗Graves眼病的疗效和安全性。方法　将 36例Graves眼病的患者分为两组 ,A组 18例采用泼尼松 (完成治疗者 16例 ) ;B组 18例采用环孢素A与泼尼松、水飞蓟素联合治疗。疗程均为 3个月。疗效判断采用Given wilson积分指数系统和美国甲状腺疾病协会分级标准。结果　 3个月后 ,A组 16例中 9例 (5 6 2 % )好转 ,7例 (4 3 8% )无变化 ;B组 18例 ,其中12例 (6 6 7% )好转 ,6例 (33 3% )无变化 ;B组的好转率高于泼尼松组。各组治疗后眼病分值均有一定程度下降 ,下降幅度以B组较明显。A组中有 3例血糖增高 ,2例退出治疗 ,B组中未发现有明确的药物不良反应。结论　免疫抑制剂环孢素A与泼尼松、水飞蓟素联合治疗Graves眼病 ,其疗效优于单用泼尼松 ,且不良反应较少。Objective To study the efficacy and tolerability of cyclosporin A(CsA),prednisone and silymarin combination method with prednisone contrast in order to look for Graves' ophthalmopathy(GO) treatment more effectively with little side effect.Methods 36 untreated GO patients were enrolled in the study,they were divided into group A and group B,the group A(n=18) treated with prednisone(16 examples completed the treatment).The group B treated with cyclosporin A,prednisone and silymarin for 3 months.The disease sererity and therapeutic reponse were quantitatively assessed with ophthalmopathy system from idex scoring according Given-wilson and the ratings standard by the thyroid gland disease association of the United States.Results After 3 months therapy,9(56.2%) of the 16 patients treated with prednison improved;In the group B,12(66.7%) of the 18 patients improved.The group B showed more effective than the group A in the treatment of GO.3 patients developed impaired glucose tolerance in group A,there were two patients who ceased the therapy in the group A.Conclusion Cyclosporin A(CsA),prednisone and silymarin combination treatment in patients with Graves' ophthalmopathy is more effective than treated only with prednisone,moreover it had a better tolerability than the group A.
