摘  要:目的 :本研究旨在阐明B型利钠多肽 (BNP)与充血性心力衰竭 (CHF)严重程度的相关性。方法 :对 12 3例CHF病人和 34例非CHF病人进行血浆N末端BNP原 (NT proBNP)测定 ,所有病人均接受心脏彩色超声多普勒检查 ,测得左室射血分数 (LVEF)和左室舒张末期内径 (LVDED)。所得结果进行统计检验 ,以评价NT proBNP与心功能衰竭严重程度、LVEF、LVDED的关系。结果 :非CHF组与CHF组NT proBNP浓度存在显著性差异 ,分别为 35 .76± 7.4 2pg/ml和 90 2 .72±188.84pg/ml,P <0 0 5 ;CHF组中根据NYHA心功能分级划分为 4组 ,各组间NT proBNP浓度 :Ⅰ级 (n =4 4 ) 195 .1±39.0 3pg/ml、Ⅱ级 (n =35 ) 5 2 2 .4 3± 14 9.88pg/ml、Ⅲ级 (n =2 0 ) 115 5 .9± 2 92 .91pg/ml、Ⅳ级 (n =2 4 ) 344 2± 872 .79pg/ml。单因素方差分析显示各组之间有显著性差异 ,P <0 0 5 ;各组NT proBNP浓度与LVEF和LVDED经直线回归分析检验存在相关性。结论 :NT proBNP浓度检测为临床CHF的诊断。Objectve:To investigate the relationship between B type natriuretic peptide (BNP) and congestive heart failure (CHF).Methods:We enrolled 157 inpatients. The participants were divided into several subgroups for NT-proBNP analysis: patients without cardiovascular CHF (n= 34), patients with cardiovascular CHF (n = 123), NYHA class I CHF (n = 44), class II CHF (n = 35), class III CHF (n = 20), and class IV CHF (n = 24). All patients received colorful doppler ultrasonocardiogram (CDFI) to determine left ventricular ejection fraction (LVEF) and left ventricular end-dilate diameter (LVDED). The data was tested by statistical methods and P<0.05 was considered as significant difference.Results: There was statistical significance between individuals with and without CHF. The concentrations of NT-proBNP of individuals without and with CHF were 35.76±7.42pg/ml and 902.72±188.84 pg/ml respectively (P<0.05). For NYHA classification I through IV, the concentrations were 195.1± 39.03 pg/ml(n = 44), 522.43± 149.88 pg/ml(n =35), 1155.9± 292.91 pg/ml(n =20), and 3442± 872.79 pg/ml(n=24)respectively. Furthermore, the concentrations of NT-proBNP between every 2 subgroups have significant differences. Correlations between NT-proBNP and LVEF or LVDED were significant by liner regressive analysis.Conclusion:Determination of NT-proBNP provides an effective and convenient way to diagnose of CHF and to evaluate the risk and prognosis of patients with CHF.
