摘 要:目的 探讨原发性肝癌病人癌组织和外周血白细胞 (PBL)中多药耐药基因 (MDR1 )的表达及其相关性 ,分析其与预后的相互关系 ,寻求建立临床上简便易行的化疗方案及预后评估指标。方法 应用荧光定量—聚合酶链反应 (FQ PCR)技术和免疫组织化学方法 ,分别检测 5 0例肝癌病人及 1 5例对照组病人的外周血和肝癌组织中MDR1的表达。结果 以免疫组织化学和荧光定量PCR方法测定肝癌组织MDR1阳性表达率分别为 70 %和 6 6 % ,荧光定量PCR方法测定外周血白细胞MDR1阳性表达率为 5 2 % ,统计学分析无显著性差异。外周血MDR1表达阳性病人近期复发率为34.6 % ,表达阴性者为 1 2 .5 % ,统计学上存在显著性差异。结论 肝癌组织和外周血白细胞MDR1表达具有相关性 ,外周血MDR1表达荧光定量检测结果可作为指导化疗和评估预后的动态监测指标。Objective To explore the correlation between multidrug resistance gene 1(mdr1)expression detected in neoplastic tissues and peripheral blood leucocytes(PBL)in hepatocellular carcinoma,and analyze its significance upon prognosis,thus to establish a more convenient index to evaluate chemotherapy and prognosis.Methods Among 50 HCC patients and 15 patients of control group,the mdr1 expression in tumor tissue and peripheral blood was detected by immunohistochemistry and FQ PCR,respectively. Tumor tissue showed positive expression of mdr1 in 70% by immunohistochemistry and 66% by FQ PCR respectively,and the expression rate in PBL was 52 % by FQ PCR with the difference being not significant.The short term recurrence rate was 34.6% in the patients who were positive for the expression of mdr1 in PBL,while 12.5% in those negative for the mdr1 expression in PBL with the difference being significant.Conclusion The expression of mdr1in PBL was related with that in tumor tissue.The result of FQ PCR upon mdr1 expression in PBL is supposed to be a new kinetic index to establish individual schedule of chemotherapy and evaluate the prognosis of hepatocellular carcinoma.
