摘 要:目的 建立杂种犬肺内源性、外源性急性呼吸窘迫综合征 (ARDS)模型。方法 健康雄性杂种犬 12只随机分为肺内源性ARDS模型组和肺外源性ARDS模型组 ,每组 6只。采用静脉注射油酸形成肺外源性ARDS模型 ,应用十六烷磺基丁二酸钠盐气管内吸入形成肺内源性ARDS模型。动态观察ARDS模型犬气体交换指标和肺不同部位 (肺尖叶、肺心叶和肺膈叶 )支气管肺泡灌洗液 (BALF)中部分炎性介质 (TNF α ,IL 1β,IL 6 )的变化。 结果 肺损伤后模型犬氧合指数明显恶化 ,肺内源性ARDS模型肺尖叶和肺心叶BALF中炎性介质水平明显高于肺外源性ARDS模型。结论 本实验成功建立了肺内 /肺外不同源性ARDS模型 。Objective To establish acute respiratory distress syndrome (ARDS) induced by pulmonary and extra pulmonary causes in dogs. Methods 12 male mongrel dogs were randomly divided into ARDSp (ARDS caused by pulmonary causes) group and ARDSexp (ARDS caused by extra pulmonary causes) group. ARDSp dogs inspired detergent to cause lung injury, and ARDSexp dogs were injected oleic acid through central vein. The results of TNF α, IL 1β and IL 6 of BALF in different areas of dog's lung (upper lobe, heart lobe, and diaphragm lobe) and arterial blood gas under mechanical ventilation treatment were measured. Results After lung injury, the arterial oxygenation index was getting worse, and the concentrations of TNF α, IL 1β and IL 6 in BALF were obviously getting higher. The detected inflammatory factors in upper lobe and heart lobe of ARDSp dogs were higher than ARDSexp dogs. Conclusion The dogs in the ARDSp and ARDSexp were accorded with the standard of ARDS models, and inflammatory factor releasing in different lung areas showed the statistical differences between ARDSp and ARDSexp dogs.
