应用纯化抗原检测尿液和血清抗幽门螺杆菌抗体的比较

摘　　要：目的 :对幽门螺杆菌 (Helicobacterpylori,Hp)感染患者尿液和血清抗Hp抗体进行比较研究 ,探讨尿液抗Hp抗体检测的临床意义。方法 :分别以多株Hp超声上清、重组Hp热休克蛋白A亚单位 (rHspA)纯品和重组Hp热休克蛋白尿素酶B亚单位 (rUreB)纯品混合物为抗原 ,采用酶联免疫吸附试验 (ELISA)方法对 12 6例患者的尿液抗HpIgG和血清抗HpIgG、抗HpIgA同时进行检测。结果 :使用rHspA和rUreB混合纯品进行检测 ,其特异性和敏感性明显优于Hp超声上清。诊断Hp感染的优劣顺序为 :血清IgG、尿液IgG、血清IgA。结论 :尿液的抗体检测可用于Hp感染诊断 ,尤其是尿液抗HpIgG是一个诊断Hp感染的敏感、特异指标。Objective To investigate the urinary antibodies of Helicobacter pylori(Hp) and its clinical significance.Methods Supersonic supernatant of whole-cell Hp and mixture of purified recombinant Heat Shock Protein A(rHspA) and purified recombinant Urease B(rUreB) were used as antigens to detect anti-Hp-IgG and IgA antibodies in serum and urine in 126 patients by ELISA.Results The sensitivity and specificity of mixture of purified rHspA and rUreB were superior to spuersonic supernatant. The serum anti-Hp IgG had the highest accuracy in Hp infection diagnosis , second was urinary anti-Hp IgG , and then serum anti-Hp IgA and urinary anti-Hp IgA.Conclusion Urinary anti-Hp IgG is a sensitive and specific marker for Hp infection diagnosis.
