SARS恢复期肺纤维化中西医结合诊治的研究

摘　　要：目的 :探讨中西医结合方法对恢复期严重急性呼吸综合征 (SARS)患者肺纤维化的治疗作用。方法 :2 0 0 3年 6月开始 ,采取随机对照前瞻性研究方法 ,将筛选出的 36例肺部 CT提示有纤维增生和纤维化的患者随机分为单纯中药组 17例和中西医结合组 19例。单纯中药组给予中药复方 86 1冲剂口服 ,中西医结合组在服用中药复方 86 1冲剂的同时加用西药富露施 (N 乙酰半胱氨酸 )。于服药后 1、3个月复查患者首次检查不正常的血清学、病源学及免疫学各项指标 ,并观察胸部 CT的病变及纤维增生情况。结果 :两组抗纤维化治疗的总有效率分别为 6 0 .0 % (9/ 15例 )和 6 4 .7% (11/ 17例 ) ,两组之间无显著性差异。结论 :单纯中药复方 86 1冲剂治疗 SARS恢复期引起的肺间质纤维化有效 ,可减少西药的使用 ,减轻患者的经济负担。Objective: To investigate the treatment effects of integrated traditional Chinese and weste rn medicine(TCM WM) on lung fibrosis in recovery stage in patients with severe a cute respiratory syndrome(SARS). Methods: From June of 2003, a randomized, placebocontrolled, prospective clin ical trial was conducted. Thirtysix cases with fiber hyperplasia and pulmonary interstitial fibrosis showed by CT were randomly divided into two groups. Seventeen c ases of the pure traditional Chinese medicine treatment group were treated with traditional Chinese herb com posite 861 medicinal granules(复方861冲剂) orally, and 19 cases of TCM WM gr oup were treated w ith composite 861 medicinal granules and Nacetylcysteine simultaneously. The abnormal indexes of ser ology, etiolog y and immun ology examed at the first time were measured at 1 st and 3 rd month after treatment, and the changes of fiber hyperplasia were observed by pulmonary C T. Results: The total efficiency rate of antifibrosis treatment in two groups we re 600%( 9/15 cases) and 647%(11/17 cases), respectively. There was no si gnificant diffe rence between two groups. Conclusion: The treatment of Chinese herbal composite 861 medicinal granules has the e ffect of reducing the use of western medicine, decreasing the cost for disea se.
