超早期手术结合中药治疗高血压脑出血的临床研究

摘　　要：目的 :寻求能提高高血压脑出血临床疗效的救治方案。方法 :采用随机对照试验方法对 72例高血压脑出血患者进行分组治疗研究。治疗组 32例 ,采用中西医结合方案 (包括超早期手术 ,常规西医、西药及中药清开灵注射液静脉滴注等综合救治方案 ) ;对照组 4 0例 ,采用手术 ,常规西医、西药治疗。连续治疗 2 8d后进行疗效评定。结果 :治疗组临床疗效明显优于对照组 (χ2 =4 .15 ,P<0 .0 5 ) ,患者死亡数明显减少 ,差异有显著性(χ2 =8.0 4 ,P<0 .0 5 ) ;并发症发生数治疗组少于对照组 (肺部感染 6∶ 16 ,χ2 =11.37,P=0 .0 0 1;上消化道出血8∶ 16 ,χ2 =4 .10 ,P=0 .0 4 )。结论 :超早期手术结合中药治疗高血压脑出血疗效较不用中药组为好。Objective: To look for an eff ective treatment for hypertensive intracerebral hemorrhage. Methods: By the method of random and control, patients with hypertensive intracerebral he morrhage were randomly divided into two groups. The treat ment group(32 patients) was treated with integrated traditional Chinese and we stern medicine therapy, including superearly operation, conventiona l western medicine and Qingkailing injection treatment. The contro l group (40 patien ts) was treated with operation and conventional western medicine treatment. The effect was evaluated on 28 th day after treatment. Results: The effect of the treatment group was superior to that of the control group(χ 2=415, P<005). The treatment group had lower mortality rat e (χ2=804,P <005) and lower morbidity of complications( pulmonary infection: 6/16 cases, χ2=1137, P=0001; uppercongestive hemorrhage: 8/16 c ases, χ2=410, P=004), statistical data indicated that there was significant difference between treatment group and control group. Conclusion: Superearly operation with traditional Chinese medicine in treatment of hypertensive intracerebral hemorrhage has a better effect than the treatm ent without tradit ional Chinese medicine.
