摘 要:目的:研究治疗剂量条件下,滤泡状甲状腺癌转移灶的131I代谢规律。方法:采用γ照相机对滤泡状甲状腺癌患者癌转移灶行1~8d、16d、24d的图像采集,观察转移灶内每个象素放射性计数随时间变化的规律,并通过曲线拟合求解代谢参数。结果:滤泡状甲状腺癌转移灶131I代谢均呈单指数代谢规律。分化型滤泡状甲状腺癌不同部位的骨转移灶代谢速率相近;不同器官(肺、骨)的转移灶代谢速率有差异,骨转移灶快于肺转移灶。低分化的滤泡状甲状腺癌转移灶131I代谢速率快,主要表现为生物代谢。结论:转移灶的131I代谢速率能够反映病灶的生物学特性,是定量研究甲状腺癌转移灶较好的指标;也可作为病灶治疗疗效的参考指标。Objective :To find out 131 I metabolic regularity in metastatic lesions of follicular dif-ferentiated thyroid cancer at the therapeutic dose.Methods:Twenty six patients with follic ular differan-tiated thyroid cancer had undergone a surgical thyroidectomy pri or to the treatment.The counts/pixel on metastatic mass was measured with?ca mera(pho/gamma HP model)equipped with GCCS-89imag-ing collecting and an alyzing system at1~8days,16and24days respectively after administration o f therapeutic dose of 131 I.By fitting the time-radioactivity curve depending on data of measuremen ts on ROI,the metabolic equation and parameters were established.Results:13 1 I metabolism in the metastatic foci of follicular differentiated thyroid car cinoma (FDTC)decays in monoexponential role.First,131 I metabolic speed of follicular differentiated thyroid carcinoma in differe nt bone sites was approximately the same.Second,the 131 I metabolic speed of follicular differentiated thyroid carcinoma focuses in dif-ferent organs was not identical.Lung metastatic foci decays slower than bone,which could be related to better therapeutic effect in pulmonary lesion th an in bone lesion in clinic practice.Third,the 131 I metabolism in metastatic focus of poorly differen tiated follicular carcin oma showed lower 131 I uptake a-bility,and 131 I metabolic speed was quick and mainly showed biological metabolism.And the last,in the metastatic lesion,the lower 131 I metabolic speed,the better the therapeutic effect.The faster 131 I metabolic speed,the wors e the therapeutic effect.Conclusions:131 I metabolic speed in the metastatic foci offers an ideal method to quantitat ively study the 131 I treatment of patients with follicular differenti-ated thyroid carcinoma .It can reflect the inner biological characteristics of the metastatic lesion s and is a more valuable index for evaluating therapeutic effect of 131 I therapy.It is helpful for precisely calcu-lating radiation absorb tion dose of the metastatic foci and quantitative ra diation dosimetric
