老年性白内障表面麻醉下颞侧透明角膜切口超声乳化及人工晶体植入术

摘　　要：目的 :观察表面麻醉下颞侧透明角膜切口超声乳化白内障摘除及人工晶体植入术的临床效果。方法 :61 6例 ( 645只眼 ) ,表面麻醉下用瑞士产Meyco钻石刀经颞侧透明角膜做一内口 2 .8mm ,外口3.2mm ,隧道长 1 .75mm的切口 ,在切口上方 30度左右角膜做一辅助切口 ,前房内注入透明质酸钠 ,做 5 .5mm连续环形撕囊 ,撕囊过程中出现放射撕裂者 ,用“开罐式”破囊完成余下前囊膜的切开 ,囊内用拦截劈裂法将晶体核分裂乳化吸出后 ,吸出残余皮质 ,植入人工晶体 ,切口自闭 ,术后涂典必殊眼药膏于结膜囊内。结果 :术后视力≥ 0 .5者 1d后占 63.67%,1W 69.79%,1月 77.1 4 %,2月 87.78%;术后角膜散光 ,术前角膜散光 ( 0 .75± 0 .5 8)D ,术后 1W、1月、2月 ,分别为 ( 0 .85± 0 .75 )D、( 0 .82± 0 .5 6)D、( 0 .77± 0 .5 4 )D ;术前角膜散光与术后 1周、1月、2月均无显著差异 ,P>0 .0 5。结论 :此术式与巩膜隧道切口具有相同的术后效果 ,是一种较为理想的术式。Objective:To investigate the therapeutic effects of phacoemulsification and intraocular lens implantation through atemporal clear tunnel incision with surface anesthesia in the elderly. Methods: Six hundreds and sixteen senile cataract patients (645.eyes)under went phacoemulsification and intraocular lens implantation through a temporal clear corneal tunnel incision with surface anesthesia which has an internal opening of 2.80mm and external opening of 3.2mm tunnel,tength of 1.75mm. The postoperative uncorrected visualacuity the changes of corneal astigmatism were observed Results:The proportions of uncorrected visnal acuity of 0.5 or better were 63.67%, 69.79%,77.14%, 78.78%.respectively at one day.One week, one month and two months after the operation. The mean preoperative astigmatism was (0.75±0.58)D;There was no statistical difference (P>0.05).between the mean preoperative astigmatism and postoperative astigmatism at one week and one month; two months,which were respectively (0.85±0.75)D,(0.82±0.56)D, (0.77±0.54)D; Condusion:The phacoemulsification and intraocular lens implantation through a temporal clear corneal tunnel incision with surface anesthesia in the elderly and scleral tunnel are same result .The means is more convenient and effective and reduce pain when the operator skilled .
